AFFIDAVIT

I, JAMES ARNEL M. DUAVE, of legal age, Filipino, single and a

resident of Purok 1, Balogo, Sorsogon City after having been sworn to in
accordance with law do hereby depose and state:

1. That I have no pending case at present, neither I have criminal

records before;

correct to the best of my knowledge and belief, and for all legal
intents and purposes this may serve;
