We are pleased to renew your appointment for the position of CONSULTANT under the

Pre-Employment Requirements
‘The following documents must be submitted on or before the first day of work, namely:
‘a. Personal Data Sheet (fully accomplished);
b. Two 2) sets of 2"x2” ID picture; and
& Photocopy of atleast two (2) valid government issued ID.
Date of Commencement
Your renewal of employment will commence on 15 Mageh 2022 or upon renewal of the
necessary licenses and permits to work in the Philippines eg. Alien Employment Permit
(AEP) issued by the Department of Labor and Employment (DOLE) and shall be for a
period of one (1) year unless renewed by the company.
Compensation and Benefits
Your basic salary is Php 80,000.00 per month, subject to statutory deductions (SD) and
exclusive of de minimir benefits (DMB).
Subject to any applicable law and with your express consent, the company reserves the tight
to deduct from your pay any sum which you may owe including, without imitation, any over
payment of salary ot for advances or loans made to you by the company, or for damage or
loss of goods expressly entrusted to you for custody or for loss of money for which you are
required to account, where the damage or loss is directly attributed to your negligence or fault

With your express consent, the company shall deduct from your salary all such sums itis
‘authorized to deduct under the laws of the Republic of the Philippines, whether for
mandatory contiibations, withholding tax or otherwise.
Duties and Responsibilities
Summary of your duties and responsibilities are as follows:
& Give advise on competency standard, capability taining and talent
development:
b. Analyze the company’s data and information;
& Assist in the development of the company’s management plan and
operating procedures;
4. Give advice on risk identification and management;
Provide reports and recommendation for the company’s management
decisions.
£ Provide support for client’ inquiries and maintain professional business
relationships with existing and future clients.
Your regular place of work shall be at Unit 903, 139 Corporate Center, 139 Valero Street,
Saleedo Village, Makati City 1227, You expressly agree that the said place of work may be
changed by the company without loss of seniority rights, continuity of tenure and employment
status, but such change shall be limited within the Philippines.
You may be tasked to go on out-of-town trips to perform your duties and responsibilities
from time to time, with prior instructions and provisions for travel and accommodation, when
necessary.
Working Hours
The company’s working hours are Monday to Friday from 9:00 pm 0 6:00 pm. You are
required to repott at the office during the said working hours, except when you are otherwise
authorized to go on fieldwork to aitend conferences, meetings, of any other official business
‘You are entitled to one (1) hour of lunch break everyday and a fifteen (15) minute break in
the morning and in the afternoon.
‘You may be required to work beyond the normal working hours depending on the exigencies
of the work at the sole discretion of the company.

‘You may be required to attend prescribed training course/s as may be deemed necessary by
the company for the effective performance of your duties,
Leave of Absence
All leave applications should be filed at Ieast three (3) days prior to the intended leave and
shall be approved at the discretion of the company, except for sick or emergency leaves, in
which case the leave application may be filed within twenty-four (24) hours upon reporting
back to work and in every case with supporting certificates and/or documents regarding the
cause of such leave.
Applications for leave, whether paid or unpaid, shall be granted at the sole discretion of the
company.
Upon completion of one (1) year service from the date of commencement of this
appointment, you shall be entitled to the statutory five (5) day Service Incentive Leave (SIL),
application of which shall be subject to the provisions above-provided. Unused SIL shall be
convertible to cash on the anniversary date of your employment.
Additional paid leave credits (sick and vacation leave) shall be given at the discretion of the
company in consideration of your length of service.
Conflict of Interest
Upon your appointment, you are required to devote the whole of your time, attention and
abilities during your hours of work to your duties and zesponsibilites.
You may not, whether directly or indirectly, undectake any work or duty for or be concemmed
or interested in any office, fim or business or person, whether within or outside your hours
of work, except with the express consent of the company.
‘Termination and Notice
Either party may voluntasiy terminate this appointment before the expiration of the
probationary petiod provided herein by serving to the other party written notice of such
intention at least thirty (30) days prior to the effectivity date of such termination.
Subject to applicable laws and regulations, you may be terminated with immediate effect for
any of the following reasons:
a. Making any false claim or misrepresentation as to your qualifications,
experience or previous employment, oF if in any other way you provided

b, Committing any act of misconduct, willful disobedience or breach of
your obligations under the terms of your employment;
«Having been found guilty of any conduct which in the opinion of the
company brings you or the company into disrepute;
4d. Dispiay aggressive behavior, or commit any physical act of abuse, assault
or violence against others,
€ You are unfit for work due to the influence of alcohol or prohibited
drugs;
£ Committing any act of dishonesty including but not limited to theft or
cheating:
& Conviction of any criminal offense punishable with imprisonment or
involving moral turpitude;
h, Becoming of unsound mind;
i. allure to catry out your duties and responsibilities up to a reasonable
standard acceptable io the company;
j. Having been found guilty of any default or neglect in the discharge of
your duties or responsibilities;
k. Violation of any of the company’s Rules, Regulations and Policies, which
‘warrants the penalty of termination;
|. Hlaving been found to be suffering from any disease and your continued
czmployment is prohibited by law or is prejudicial to your health as well
as to the health of your co-workers; and
m. For any of the just and authotized causes mentioned in the Labor Code
of the Philippines.
Upon the termination of your employment, you undertake to return to the company all
documents, records, items and matetials in your possession o: custody belonging to the
company of to its clients, and you farther undertake not to retain any copies (including
electronic or soft copies) thereof.
In case of termination 2s provided herein, your last pay shall be withheld until such time that
office documents and/or propery in your possession have been surrendered and the
necessary cleatance is issued by your immediate cupervisor,
Confidentiality and Privilege
You shall not disclose to any third party eny information obtained during your cousse of
‘employment, either from within the company or from its clients, unless the disclosure of such
information is with the express consent of the client concemed and the company, or otherwise
required by law.
‘The obligation to maintain confidentiality shall apply after your employment.10.3. You shall not destroy, make copies, duplicate or reproduce, in any manner whatsoever, any
document or information obtained during your employment without the express consent of
the company
104, If requested by the company, you undertake to delete any confidential information from any
reusable material and destroy ll other documents (including copies thereof) and tangible items
‘which may contain or refer to any confidential information which ate in your possession oF
under your control, and ia any event, retum to the company upon termination of your
employment for any reason whatsoever, all materials, documents, copies of such documents
‘and tangible ites which belongs to the company and/ot its clients.
1, Miscellaneous Provisions
11.1. You undertake to observe all Policies and Procedures of the company that may be applicable
to your position, and you agree that the company reserves the right to amend or issue new
Policies and Procedures from time to time, to which you likewise undertake to observe.
11.2. The terms and conditions set out in the Policies and Procedures of the company shall be
deemed to form part of the terms and conditions of your appointment.
Should you find the foregoing terms and conditions acceptable, kindly indicate your

appointment on or before the date of your employment
