We, Caren Dangembro and Eldridge Roxas are both alumnae of Occidental Mindoro State
College, we are set to get married on March 12, 2022.

I write to formally ask permission the use of some location in the Occidental Mindoro State
College – Mamburao Campus for the purposes of using it as a venue for our pre-nuptial
photoshoot on {Date and time of Use}. The School grounds is a witness of our humble
beginnings as young couple and we would like to pay homage to the institution.

Our production team includes, 3 photographers and 2 makeup artists that are all fully
vaccinated. We will follow safety health protocols while on shoot and ensure that the school
premises is left in perfect condition.
