1.0 Scope

1.1 This procedure defines the requirements and method used by ESC – Environmental Solutions and
Consultancy in the activity of inspection, testing and certification of Elevator.

2.0 Purpose

2.1 This procedure tends to protect those who use and service the equipment with applying the code of
standard requirements and local regulation.

3.0 Pre-Examination Requirements

3.1 The equipments needed as follows (but not limited to the following);

3.1.3 Copy of Equipment Operational Manual
3.1.4 Copy of Operator or Maintenance Manual

4.0Site Risk Assessment


4.1 The inspector aims to work with the maintenance to ensure the elevator can be inspected safely and
effectively, and should cover the following (but not limited):
4.1.1 Assessment of safe access and egress shall be utilized before entering the machine room,
car top and pit.
4.1.2 Use personal protective equipment, e.g. safety shoes, safety gloves, helmet, visibility vest,
etc.

4.1.3 Lift entrance shall be blocked and warning sign on ground floor shall be posted to avoid
unauthorized person will enter inside the lift while inspection is going on.
4.1.4 A means to isolate the device from sources of power.

4.2 Periodic Thorough Inspection


MACHINE ROOM (but not limited to the following);
4.2.1 In the machine room, almost safety devices of the elevator are possible to test.
4.2.2 Undertake testing works in accordance with EN 81-20. This specification includes but not
limited to the following:

● Checking the machine room door installation, locks, way of opening.


Access doors

Minimum width 0.60 m

Minimum height 1.80 m

● Inspections within the machine room including safe access, floorings, ladders and fixings
(if applicable).
● Checking the lightings on working areas with an intensity of 200lux on floor level.
● Ambient temperature is assumed to be maintained between +5˚C to +40˚C.
● Checking the socket outlet provision and their operations.
● Checking the clearances in front of the control cabinets;

Minimum depth 0.70 m

Minimum width 0.50 m of the full width of cabinet

0.5m x 0.6m minimum in front of moving and maintenance part

● Checking the clear height inside the machine room with a dimension of 2 m at working
areas.
● Checking the clear vertical distance of at least 0.30 m above the rotating parts of the
machine.
● Checking the dimension of holes in the slab and room floor that shall be reduced to a
minimum for their purpose.
● All rope holes shall be provided with ferules with a height of 50mm above the slab or
finished floor.
● Visually check the hand wheel device (for manual rescue operation) that shall be smooth
spoke less wheel.
● Checking the main rope termination for any mechanical contact, corrosion and worn out.
Rope termination shall be on following;
- Self-tightened wedge type sockets
- Ferrule secured eyes
- New; swage terminal
● Checking the balustrade on machine platform to prevent accidental fall.
● Checking any other equipment installed that are not related to the lift operations.
● Checking the condition of the fire preventive equipment.
● Checking the following notices and markings below;
- Manual rescue operation procedure
- Restriction sign outside the machine room door
- Electrical hazards sign
- Tripping, bumping sign
- Suspension rope markings
- Warning notice to the maintenance and inspection personnel
- Lift parts identifications
- Lifting hooks load capacity
- Lift parts specification data

4.3 ELECTRICAL INSPECTION

4.3.1 Checking the condition and location of main supply for safe access.
4.3.2 Main switches and cables shall be checked to ensure that they are suitable rated as per
manufacturer drawings.
4.3.3 Checking the main switch that shall be lockable in off position.
4.3.4 Checking the RCD (residual current device) by tripping the test button and after the test,
the tripping mechanism remains free.
4.3.5 Checking the operation and condition of phase protection device (if fitted).
4.3.6 Checking the separation of supply between socket outlet and car lighting from the machine
unit.
4.3.7 Load measurement of three phase conductor and single phase conductor.
4.3.8 Checking that isolating devices, circuit breakers and fuses are not shorted out by links or
wires.
4.3.9 Visual examination inside the controller components for loosed wiring terminations,
labeling and protected against accidental contact.

4.4 ELECTRICAL EMERGENCY OPERATION


4.4.1 Checking the automatic rescue device by bringing the car to the most top floor level and
giving a call in down direction and switch off the main switch. Check if the battery drive is
capable of bringing the car to the floor level and open to respective floor.
4.4.2 Check the emergency lighting inside car. It shall have 5 lux for 1 hour @ alarm button and 1
m from floor in center of car.
4.4.3 With the main switch off, alarm device and intercom shall be functioning and connected to
the Machine room/building maintenance room.
4.4.4 Keep the ELD switched off after the procedure.

4.5 MAIN TRACTION SHEAVE / PULLEY


4.5.1 Checking the condition of main traction sheave and deflector pulley they shall be provided
by a protection cover against bodily injury and protection bar against rope leaving the
pulley if rope slack.

4.5.2 Traction sheave/Deflection pulley diameter and compare with the measured rope
diameter.
- The ratio between the pitch diameter of sheaves, pulley and the nominal diameter of
the suspension ropes shall be at least 40, regardless of the number of the strands of the
suspension ropes.
4.5.3 Visually check the groove condition for any worn-out, dirt and corrosions.

4.6 MACHINE BRAKE


4.6.1 Thoroughly check the brake shoe fixing for any worn, corrosions and crack.
4.6.2 Checking the two independent brake shoes are provided.
4.6.3 Each brake shoe separately is able to stop the car while moving downwards loaded 100%
the test is similar running upward empty due to weight of the counterweight.

4.7 ROPES
4.7.1 Thoroughly check and take in consideration the following points;
● Rope termination at car and counterweight side.
● Rope condition.

4.8 OVER-SPEED GOVERNOR (ascending and descending)


4.8.1 Visually check the fixing condition of over-speed governor.
4.8.2 Functionally examined the free mechanical engagement of over-speed governor.
4.8.3 Functionally examined the electric safety switch of over-speed governor.

4.8.4 Checking the speed (shall not more than 5% of the rated speed and not lower than 8% of
the rated speed).

4.9 UNINTENDED CAR MOVEMENT (if fitted)


4.9.1 Checking the operation of protection against car unintended movement. Bring the lift to the
floor level in normal operation. While the door is in open condition, manually released the
machine brake and confirm that the safety device is working according to its function.
4.9.2 In addition, the Lifts shall be provided with a means to stop unintended car movement
away from the landing with the landing door not in the locked position and the car door
not in the closed position, as a result of failure in any single component of the lift machine
or drive control system upon which the safe movement of the car depends, except failure of
the suspension ropes or chains and the traction sheave or drum or sprockets of the
machine, flexible hoses, steel piping and cylinder.

4.10 BUFFER TEST AND LIMIT SWITCH


4.10.1 For upper limit switch functional test, bring the car to the top most level. Mark the position
on wire ropes. Switch to inspection mode and send the car down (approx. 20 cm). Bring the
car up on inspection mode until the maintenance switch cut. Mark.
4.10.2 For buffer testing, with the main switch/ELD off. Fix the hand wheel and move the car
upwards until the counterweight is fully buffered and the ropes are slipping on the traction
sheave. Mark the position on the wire ropes. Measure the distance and document the value.
Bring the car back to floor level and remove the wheel.
4.10.3 For downward limit switch test, send the car to the most bottom level. Mark the position
on wire rope. Switch to inspection mode until maintenance switch cut. Mark.
4.10.4 For buffer testing, with the main switch/ELD off. Fix the hand wheel and move the car
downwards until the counterweight is fully buffered and the ropes are slipping on the
traction sheave. Mark the position on the wire ropes. Measure the distance and document
the value. Bring the car back to floor level and remove the wheel.

4.11 TRACTION TEST


4.11.1 For traction test, the car shall be maintained at floor level without slippage when loaded
125%.
4.11.2 For traction test, it shall be ensured that any emergency braking causes the car, whether
empty or with rated load, to decelerate with value not exceeding the setting of the buffer,
including stroke buffer;
4.11.3 If either the car or the counterweight is stalled (e.g. on the buffers) the following
requirement shall comply;
- The ropes shall slip on the traction sheave and not allow the car or counterweight to be
raised.
- The lift drive system shall not allow the empty car or the counterweight to be raised.

4.12 LOAD TESTING


4.12.1 A physical test weights to examine the safe working load and load control device of the lift
shall be needed on the inspection, by placing inside the lift. Test equivalent to maximum
safe working load and overload is 110 % of rated load. When the overload condition, the
elevator will stay in the same floor level and the doors will remains open position. The
overload indicator and the overload buzzer will be ringing.
NOTE: The examination of load test shall be conducted at lowest floor level.

4.13 BALANCING BETWEEN CAR AND COUNTERWEIGHT


4.13.1 Ask for the previous balancing report. Send the car and the counterweight at the same floor
level on the shaft. Calculate the weight that must be loaded to the cabin to achieve proper
balancing. Main switch shall be off. Manually operate the hand wheel up to a point where
it is freely moving. Considerations on car interior fixings shall be taken into account.

4.14 SAFETY GEAR TEST


4.14.1 Send the car down in normal mode and trip the overspeed governor. Bypass the safety gear
overspeed governor switch and move down in inspection mode to make sure that proper
engagement occurred while the car is descending. The required load shall be uniformly
distributed over the area and the machine running until the ropes slip or become slack.

4.15 CAR ROOF


4.15.1 Before accessing the car roof, each safety device shall be verified functional (1) make sure
that car door switch is functioning, (2) car roof multiple emergency stop switch shall be
verified one by one, (3) inspection priority switch shall be verified also. Access the car roof
while at step height max 25 cm above or below floor level.
4.15.2 Inspection of car roof for the following requirements;
● A toe board of 100 mm height on the perimeter of the car roof
● The car roof shall be provided a balustrade with the following requirements:
- The balustrade at the access side(s) shall provide safe and easy access to the car
roof;
- It shall consist of a handrail of intermediate bar at half the height of the
balustrade;
- Considering the free distance in a horizontal plane beyond the outer edge of
the handrail of the balustrade (see Figure 11), its height shall be at least:
- (0,70 m where the free distance is up to 0,50 m)
- (1,10 m where the free distance exceeds 0,50 m)
- The balustrade shall be located within 0,15 m maximum of the edges of the car
roof;
● The horizontal distance between the outer edge of the handrail and any part in the
well (counterweight or balancing weight, switches, rails, brackets, etc.) shall be at
least 0,10 m.

● In the case of a balustrade on the car roof and independently of its height, a
warning sign or notice about the danger of leaning over or stepping beyond the
balustrade shall be fixed to each section of it. Handrail shall be provided with
intermediate bar at half the height of the balustrade
4.15.3 Functional test of inspection control station.
4.15.4 Lightings inside the lift well should be measured;
Lighting requirements on well (min.)
● 50 lux @ 1m from car roof.
● 50 lux @ 1m from pit floor.
● 20 lux at all other areas excluding shadows.

4.15.5 Checking the governor rope end termination.


4.15.6 Visually check the entire governor rope for any broken wires, strand.
4.15.7 Functional test of safety gear electric safety device.
4.15.8 Checking the compensation chain/rope termination.
● ≤ 3 m/s other compensation means than ropes can be used
● ≥ 3 m/s only compensation ropes with tensioning device shall be used
● 3,5 m/s only additionally anti-rebound device has to be provided
● Other compensation means than ropes
- min. safety factor 5
- 1,75 m/s guiding at vicinity of loop
4.15.9 Checking the fixing condition of guiderails for any loosed bolt, broken bracket, cleanliness,
joints etc..
4.15.10 Checking the condition of trailing cables.
4.15.11 Any deflector pulleys are guarded.
● 3 rope retainers if wrap angle > 120° and > 60° are below horizontal axis
● 2 rope retainers for other cases
4.15.12 Measuring the head room clearances as per standard requirements on. By bringing the car
to the topmost floor level and measure the following distances;
● Highest equipment mounted on the car roof and lowest part of the headroom

● Highest part of guide rail lengths (shoes, rollers rope termination) and the lowest part of
headroom.
● Safety spaces provision (either standing or crouching) corresponds accordingly with the
standard (5.2.5.7)

Note: all the measurement obtained at final limit switches (8.7) plus the jumping shall be deducted to measurements
obtained on the above said provision.

4.16 CAR
4.16.1 Functional test of in-car operating panel.
4.16.2 Functional test of car door reversal device.
4.16.3 Checking the presence of load capacity plate in-car.
4.16.4 Calculate the available area by measuring the dimension of the cabin. And compare the
obtain result to the corresponding capacity displayed at the panel. Recess/Extension on the
interior fixing must be considered.

● The assistance of rescuing passenger shall be only permitted from outside, being
provided on particular by the emergency operation.
● The dimension of the trap door provided on car roof for rescuing passenger shall
be at least 0.40m x 0.50 m.
● Emergency doors may be used in the case of adjacent cars, provided, however, that
the horizontal distance between cars does not exceed 1 m.
● Each car shall be provided with a means of determining the position of the adjacent
car to which persons will be rescued in order to allow it to be brought to a level
where rescue can take place.
● In the event of rescue a portable/movable bridge with handrails and a minimum
width of 0,5 m shall be provided.
● The bridge shall be designed to support a minimum force of 2500 N.
● This bridge shall be stored in the building where the rescue is to take place. The
use of the bridge shall be described in the instruction manual.
● If emergency doors exist, they shall measure at least 1,80 m high and 0,50 m wide.
4.16.8 In case the wall is made of glass, look for the details e.g. manufacturer, type and thickness.
● The fixing of the glass in the wall shall ensure that the glass cannot slip out of the
fixings, even when sinking.
● Car walls with glass placed lower than 1,10 m from the floor shall have a handrail
at a height between 0,90 m and 1,10 m. This handrail shall be fastened
independently from the glass.
4.16.9 Measure the gap car sill and landing door sill, car door panel and landing door panel.
● The horizontal gap between landing door sill and car door sill shall not be more
than 35mm.
● The horizontal gap between landing door/car door panel and upright shall not be
more than 6mm.
4.16.10 Identification and emergency manuals (if any).
4.16.11 For automatic car door, the closing force shall be measured and shall be comply with
following requirements;

● The effort needed to prevent the door closing shall not exceed 150 N. This
measurement shall not be made in the first third of the travel of the door;
● The kinetic energy of the landing and/or car door and the mechanical elements
which are rigidly connected to it, calculated or measured at the average closing
speed shall not exceed 10 J.
4.16.12 Measuring the car door apron that shall be at least 0.75m in height.
4.16.13 Checking the floors, ceiling and wall for any damaged that can cause hazards to the
passenger.
4.16.14 Checking the operation of car door electric safety device.
4.16.15 Checking the operation of mechanical car door restrictor (if fitted).
4.16.16 Checking the leveling accuracy of the lift by randomly on floors. The stopping accuracy of
the car shall be ± 10 mm and a levelling accuracy of ± 20 mm shall be maintained. If, during
e.g. loading and unloading phases, the value of 20 mm is exceeded, it shall be corrected.
4.16.17 Check and measure the running clearances between car door sill and inner surface of shaft
wall facing the car entrance. The horizontal clearance shall not be more than 150 mm.
● The distance given above:
- may be extended to 200 mm over a height not exceeding 0,50 m. There
shall not be more than one of such recesses in between two consecutive
landing doors;
- may be extended to 200 mm throughout the travel on goods passenger lifts
in which the landing doors are vertically sliding;
- is not limited if the car is provided with a mechanically locked door, which
can only be opened in the unlocking zone of a landing door.
4.16.18 Checking for any unussual vibration or noise while running.
4.16.19 Checking the operation of the lift during fire condition. In this case the lift landing doors
participating to the fire protection of the building, they shall be closed and locked in
normal operation, after the necessary period of time, which may be defined according to
the traffic using the lift, in the absence of a command for the movement of the car.

4.17 LANDING DOOR

4.17.1 Checking the effectiveness of landing door locking device. The effective locking of the
landing door in the closed position shall precede the movement of the car. The car shall not
be able to start until the locking elements are engaged by at least 7 mm.
4.17.2 Checking the landing door unlocking device. The device shall be triangle unlocking (as per
EN 81-20).
● Each of the landing doors shall be capable of being unlocked from the outside with
the aid of a key, which will fit the unlocking triangle.
● The position of unlocking triangle can be on the door panel or frame. When in a
vertical plane, on the door panel or frame, the position of the unlocking triangle
shall not exceed 2,00 m in height above the landing.
● If the unlocking triangle is on the frame and the key hole downwards in the
horizontal plane the maximum height of the unlocking triangle hole shall be 2,70
m. The length of the unlocking triangle key shall be at least equal to the height of
the door minus 2,20 m. Emergency unlocking shall be possible without making use
of climbing means such as stool, chair or ladder.
● After an emergency unlocking, the locking device shall not be able to remain in the
unlocked position with the landing door closed.
● In the case of landing doors driven by the car door, if the landing door becomes
open for whatever reason when the car is outside the unlocking zone, a device
(either weight or springs) shall ensure automatic closing and locking of the landing
door.
● If there is no access door to the pit, other than the landing door, and if the door
lock is not reachable safely within a maximum horizontal distance of 1,0 m from
the permanent access means according to a permanent installed device shall allow
a person in the pit to unlock the door.
4.17.3 Checking the operation of landing door electric safety device. The operation shall be
comply on the following requirements;
● Each landing door shall be provided with an electric safety device proving the
closed position.

● In the case of horizontally sliding landing doors, coupled with car doors, this
device may be in common with the device for proving the locked condition,
provided that it is dependent upon the effective closing of the landing door.
4.17.4 Checking the condition of landing door apron for any rust, deformation and loosed.
4.17.5 Checking the operational clearances of landing doors.
● When closed, the clearance between door panels, or between panels and uprights,
lintels or sills, shall not exceed 6 mm. This value, due to wear, may reach 10 mm.
These clearances are measured at the back of recesses, if present. Exception is made
for vertically sliding doors.
4.17.6 Checking the condition and operation of landing hall buttons and their indicators.
4.17.7 Checking the operation of landing door self closing mechanism.

4.18 PIT
4.18.1 Checking the operation of the stop switch in lift pit.
● The stopping device shall be easliy accessibel from the landing door entrance and
from pit floor.
● The stopping device shall be marked “stop”.
● They shall be bi-stable and such that a return to service cannot result from an
involuntary action.
4.18.2 Checking the pit ladder condition. Type of pit ladder on below mentioned;
● A fixed ladder, which stands upright in one position for both use and storage
purposes, or;
● A retractable ladder, which stands upright in two positions, one for use, other for
storage. The use position is obtained when a person is placing their weight on the
rung, or;
● A retractable ladder, which stands upright for storage and is manually put in
position of use by horizontal sliding of its bottom part, or;
● A movable ladder, which stands upright for storage and is manually put in an
inclined position of use, or;

● A movable ladder, which lays down on the pit floor for storage and is manually
put in an inclined position of use, or;
● A foldable ladder, which is stored in the pit and then positioned and hooked onto
the landing door sill.
4.18.3 Pit ladder requirements are as follows;
● According to the type of pit ladder chosen when designing a lift installation the
ladder shall be permanently stored in the lift pit so that it cannot be removed from
the well or used for other purposes.
● Able to withstand the weight of one person counting for 1500 N;
● Made of aluminium or steel. In the case of steel, an anti-corrosion protection shall
be applied. Wooden made ladder shall not be used.
● The length of the ladder shall be such that in position of use the length of the
uprights extends to a minimum height of 1,10 m measured vertically above the
landing sill;
The cross-section of the ladder uprights shall be so that:
● For easy and safe hand grasping, the width does not exceed 35 mm, and depth 100
mm, and
The ladder rungs shall fulfilled the following requirements:
● The clear width of the ladder rungs shall be minimum 280 mm;
● The rungs shall be equally spaced, between 250 mm and 300 mm;
● The cross section of the ladder rungs shall be either circular or polygonal (square or
more than 4 sides) with a diameter or a flat tread of minimum 25 mm and
maximum 35 mm;
● The surface conditions of the rungs shall be non slippery, i.e. by means of profiled
surface or special durable anti slippery coating.
● There shall be a clear distance of 200 mm minimum between back of any rung and
wall of the pit in the case of vertical ladder;
● The distance between the edge of the landing entrance and the ladder in its stored
position shall not be more than 800 mm;

● The distance between the edge of the landing entrance and middle of rungs shall
be 600 mm maximum for easy reach;
● The height of the first rung of the ladder shall be positioned as close as possible at
the same level as the landing sill.
4.18.4 Checking the operation of the socket outlet in lift pit.
4.18.5 Checking the condition and type of buffer.
● If buffer is fixed to the car the impact area in lift pit shall be provided by an
obstacle (pedestal) with a height of not less than 300 mm. in addition, an obstacle is
not required for buffer(s) fixed to the counterweight where a screen guard is
extended to not more than 100 mm above the pit floor.
● Energy accumulation type buffers, with linear and non linear characteristics, shall
only be used if the rated speed of the lift does not exceed 1 m/s.
● Energy dissipation type buffers can be used whatever the rated speed of the lift.
● The energy accumulation type buffers with non-linear characteristics and energy
dissipation type buffers are regarded as safety components and shall be verified
according to the requirements in prEN 81-50,
● Checking the specification of the buffer (name of the manufacturer, buffer test
certificates, specification of liquids in case of hydraulic buffer, type of buffer)
● For buffer with linear charectiristics, the total possible stroke of the buffers shall be
at least equal to twice the gravity stopping distance corresponding to 115 % of the
rated speed (0,135 v²) 7), the stroke being expressed in metres. However, the stroke
shall not be less than 65 mm.
● For buffers with linear charectiristics, shall be designed to cover the stroke under a
static load of between 2,5 times and 4 times the sum of the mass of the car and its
rated load (or the mass of the counterweight).
● For energy accumulation buffer with non linear, hitting the buffer(s) with the mass
of the car and and its rated load or of the counterweight, in case of free fall with a
speed of 115 % of the rated speed, the average retardation shall not be more than
1g;

● For energy accumulation buffer with non linear. Retardation of more than 2,5 g
shall not be longer than 0,04 s;
● For energy accumulation buffer with non linear. The return speed of the car or the
counterweight shall not exceed 1m/s;
● For energy dissipation buffers. The total possible stroke of the buffers shall be at
least equal to the gravity stopping distance corresponding to 115 % of the rated
speed (0,0674 v²), the stroke being expressed in metres.
● For energy accumulation buffer with non linear. When the slowdown of lift at the
ends of its travel is monitored, the speed at which the car (or the counterweight)
comes into contact with the buffers may be used instead of 115 % of the rated
speed, when calculating the buffer stroke. However, the stroke shall not be less
than 0,42 m.
● For energy accumulation buffer with non linear Hitting the buffer with the mass of
the car with its rated load, in case of free fall with a speed of 115 % of the rated
speed, the average retardation shall not be more than 1 g;
● Retardation of more than 2,5 g shall not be longer than 0,04 s;
● Buffers, if hydraulic, shall be so constructed that the fluid level can easily be
checked.
● Checking the condition of the counterweight screen guard.
● The screen guard shall be extended from the lowest counterweight resting fully
compressed bufferin its lowest positionto a minimum height of 2 m from the pit
floor.
● The width shall be equal to that of the counterweight.
● Checking the condition of partition between lifts.
● This partition shall extend at least from the lowest point of travel of the car, the
counterweight or the balancing weight to a height of 2,50 m above the floor of the
lowest landing.
● The partition shall extend through the full height of the well if the horizontal
distance between any balustrade and a moving part (car, counterweight or
balancing weight) of an adjacent lift is less than 0,50 m.

● This partition shall be at least the width of the moving part and extend a further
0,10 m on each side throughout the height of the well.
● Checking the condition of governor rope tension pulley and the presence of
protection cover.
● Checking the operation of the governor rope tension pulley electric safety device.
● Checking the condition of compensation rope tension pulley.
● For the lift rated speed more than 3 m/s, a compesating pulley shall be fitted.
● The ratio between the pitch diameter of the tensioning pulleys and the nominal
diameter of the compensating ropes shall be at least 30;
● The tension shall be provided by gravity.
● For the lift rated speed of more than 3.5m/s, the tension pulley shall be fitted with
anti rebound device monitored by an eletcric safety device.
● For rated speeds exceeding 1,75 m/s, compensation means without tensioning
shall be guided at the vicinity of the loop.
● Checking the refuge space in lift pit. Bring the car to the bottom most floor level
and measure the following distances;
● The clear distance between the lowest part of the car and the highest
part/equipment protruded on the pit floor (e.g. buffer). Deduct the value obtained
on every proceeding measurement.
● Safety spaces provision (either standing or laying) corresponds accordingly with
the standard

4.19 It is a Thorough Inspection that includes visual and functional excluding load test. Existing elevator
that was initially examined and commissioned before putting into service.
4.20 The inspection shall include the verification of the following (but not limited);
6.1.1 Initial Aproval
6.1.2 Design Review
6.1.3 Copy of Equipment Operational Manual
6.1.4 Copy of Operator or Maintenance Manual
6.1.5 Engineering Technical Drawings
6.1.6 Installation Procedure
6.1.7 Inspection Certificates
6.1.8 Inspection Reports
6.1.9 Rope test Certificates
6.1.10 Maintenance log

4.21 The examinations and tests shall not be more stringent than those required before the lift was the
first time put into service.
4.22 These periodical tests should not, through their repetition, cause excessive wear or impose stresses
likely to reduce the safety of the lift. This is the case in particular of the test on components such as
the safety gear and the buffers. If tests on these components are made, they shall be carried out
with empty car and at a reduced speed.
4.23 The person appointed to make the periodical test should assure himself that these components
(which do no operate in normal operation) are still in an operating condition.
7 REPORTING
The written reports shall contain the following information as a minimum:

7.1 Identify the equipment examined (serial number, make, etc), the employer and the premises;
7.2 give the date of the last thorough examination and specify when the next one should take place;
7.3 specify the safe working load of the lift;
7.4 give the reason for the thorough examination (ie following installation, according to an examination
scheme, statutory interval, etc);
7.5 identify any defect which is or may become a danger to people;

7.6 give the details of any repair, renewal or alteration required to remedy the defect and the date by which
it should be undertaken;
7.7 give details of any tests carried out;
7.8 give details of the person carrying out the report and the person validating
the report on their behalf.
If the report does not contain all the information above, you should not accept it, as this may place you in
breach of the law.

8 DEFECTIVE EQUIPMENT
If the test is not satisfactory, a responsible representative of the user shall be informed by means of
engineering report. If the test Engineer considers the defect is dangerous in nature, then arrangements must
be made by installing a warning sign “out of order” to ensure that the lift is not used until remedial work
has been carried out.

9 CERTIFICATION
When no deficiencies have been found the inspection and testing certificates will be issued. When
deficiencies are found but not detract from operating safety, the certificate maybe completed prior to
correction. Deficiencies will be reported, when deficiencies which directly affect operating safety are found,
the responsible personnel will immediately notify and the certificate will be held until the corrective action
is taken. The certificate will be reinstated only after re-inspection and testing indicates that corrections have
been made in accordance with the inspection and testing requirements.
