AFFIDAVIT OF PARENTAL TRAVEL PERMIT

I, CRISLIE CRISPIN AGULLANA, JR., of legal age, Filipino citizen and

presently residing at No. 102, Magat Salamat St., Brgy. 13, Laoag City, Ilocos Norte
with cellphone no. 0917-726-0584 after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That I am the father of CRIS WEYHNE TAGUDIN;

2. That the above-mentioned child will travel to the UNITED STATES OF

AMERICA as an immigrant;

3. That the child will leave the Philippines immediately upon approval of
her immigrant VISA and intends to reside therein;

4. That the child will be accompanied by her mother, WENDHEY ARCIBAL

TAGUDIN;

5. That I personally give my consent allowing said minor/child to travel,

together with her mother to the United States of America;

6. That I personally guarantee financial support of the child in the course of

her travel and stay outside the Philippines and further guarantee that
she will not in any manner become public ward or burden in the United
States of America or any country en route or to the Philippines .

7. That this affidavit is executed for the purpose of attesting to the truth of
the foregoing facts and for whatever legal purpose it may serve period
only.
