1.0 Scope
1.1 This procedure covers the requirements at construction site for geotechnical
investigation works and all the other work as required for soil investigation.

2.0 References
This procedure shall be maintained in accordance with the requirements of the
applicable documents as follows:
2.1 Project Contract: Construction of Ashpond Storage A5
2.2 Document Control Procedure – PQP Control of Quality Documents
2.3 Control of Inspection and Test (Site) – Inspection and Test Plan

3.0 Health Safety and Environment [HSE]


3.1 All the personnel involved in geotechnical/soil Investigation works shall
receive safety induction from SLTEC Safety Department and Clinic
3.2 A job specific tool box talk shall be given to crew members from the team
leader on daily basis.
3.3 Equipment must be checked and maintained in good condition to perform
safe work on site.
3.4 A helper must be present during all the activities in order to see if any
problem will occur.
3.5 Personnel Protective Equipment (PPE) is mandatory to be used on site
during this activity.

4.2 Driller/technician of sub-contractor and contractor shall possess adequate
knowledge on different forms of geotechnical works and its equipment.

5.0 Responsibility and Authority


5.1 Project Manager
5.1.1 Endows particular responsibilities and authorities to construction personnel.
5.1.2 Executes the construction according to the construction plan and the work
procedure.
5.1.3 Coordinate with the Owner representative, Construction Engineer, HSE
Manager/Officer, QA/QC Officer for the safe and proper execution of the work.
5.1.4 Specific attention to all safety measure and quality control in co-ordination with
HSE Manager, QA/QC Manager and in line with Approved HSE Plan and
Approved Quality Plan.
5.1.5 Prepares a construction plan and a work procedure referring to this work
instruction, which shall include the following items:
1) Owner's requirements in the specifications.
2) Work procedure, Inspection and Test standards, ITP and check lists.
3) Input plan for materials, equipment and workers.

5.2 QC Inspector / Field Engineer


5.2.1 Ensure that all the works are carried out as per the QC Plan and the related quality
procedures.
5.2.3 Inspect the completed works and requesting for the OE inspection
5.2.3 Maintain inspection and test records complete for future references.
5.2.4 Previews the quality items when creating the construction plan and the work
procedure related with this work
5.2.5 Checks whether the construction plan and the work procedure are well observed
in the actual construction.
5.2.6 Controls the Inspections and Tests related with this work

5.3 Safety Officer


5.3.1 Ensure the implementation of all safety measures in accordance with the HSE
plan and everybody should be aware of it to work with proper implementation.
5.3.2 Ensure all the implemented safety measures are adequate to maintain the
safe working on the site.
5.4 Subcontractor
5.4.1 The Subcontractor is overall responsible for the implementation of this
method statement.
5.4.2 Subcontractor shall prepare documents for notification of inspection and of the
construction record (Inspection and Test Report) in accordance with related
procedure.

5.4.3 Subcontractor shall utilize and maintain the revised documents and drawings up-
to-date for the construction work.

5.4.4 Subcontractor shall use certain forms of document and numbering system in
accordance with the RIFC's specifications.
5.4.5 Construction record (Inspection and Test Report) shall be submitted to RIFC
5.4.6 Construction record (Inspection and Test Report) shall be approved and
reviewed for its completion by relevant organizations.

6.0 General
6.1 RIFC shall provide project location plan indicating the layout of the proposed
construction. Likewise RIFC shall provide the survey and exact location of the
proposed borehole locations with clear identifying mark.
6.2 Soil investigation team shall provide investigation program which includes drilling
boreholes and collecting soil samples at desired intervals for subsequent
observation and laboratory testing to determine the capacity of soil foundation
economically and safely.
6.2.1 Soil investigation team shall record geotechnical reports such as soil profile, lab or
in-situ test results, ground water conditions and interpretation/analysis of data.
6.2.2 General Prior to any works, all personnel involved with the soil investigation will
receive an on-site induction relating to the site operations, the site rules and general
and specific health and safety obligations. Each engineers will then ensure the
individual drill crews in their area are aware of the specific requirements for each
individual hole.
6.3 Drilling works shall be stopped, in case of adverse effect such as bad visibility,
heavy rain, strong wind and etc.

6.4 Sub-Contract
Soil Investigation Team shall consist of personnel who have professional geotechnical
skills from each discipline department or each subcontractor.

6.5 Mobilization of Drilling Rig and Barricading of Works Area


Mobilization and shifting of rig through roads and side table will be conducted during
the designated period for shifting and mobilization. The area where the Borehole drilling
will be carried out shall be barricaded

7.0 Work Procedure of Soil Investigation


7.1 Soil Drilling Works
7.1.1 Existing grassland vegetation will be cut and removed in the form and topsoil will
be removed and kept separate from the general spoil.
7.1.2 Prior to the commencement of drilling activities, three (3) boreholes will be dug

to a minimum depth of 15.0 meters at the proposed borehole locations. Drilling will
be carried out using either auger boring, or, if necessary because of subsurface soil

conditions, rotary wash boring techniques.

7.2 In - Situ Testing & Disturbed Sampling


7.2.1 Before every execution of SPT test the bottom of the borehole is cleaned
carefully and then the depth of the borehole is measured also its depth.
7.2.2 SPT coupled with disturbed sampling at interval of 1 meter for the first 6.o meters
and 1.5 meters thereafter or at every change of strata using 2.0 OD and 1 3/8 OD
7.2.3 SPT samples will be immediately transferred to a heavy- duty plastic bag and
sealed to retain the natural moisture content. The bag will be marked with indelible
ink to show project title borehole number and sample.
7.2.4 The sampler shall be a two-inch outside diameter split-barrel sampler, with
an inside diameter of one and three-eighth (1-3/8) inches. The dimensions
of the sampler shall conform to those shown in the Method for
Penetration Test and Split -Barrel Sampling of Soils, ASTM Designation
D1586, the latest revision.

7.3 Rock Core Sampling


7.3.1 The individual coring runs shall be limited to a maximum length of 2.0
m. Each coring recovery shall be recorded.
7.3.2 A significant actions of the bit, water return and reasons for loss of core shall
be recorded in the boring log. Core recovery, RQD and rock logging shall
be carried out.

7.4 Ground Water Table


7.4.1 After measuring the groundwater level at completion of the borings, the bore
holes shall be suitably covered so that there will be no hazard to people, animals, or
equipment.
7.4.2 After the 24-hour groundwater level has been measured and all other
observations, records, and information have been obtained, the holes shall be filled
in such a manner as to prevent a hazard. Borings drilled through existing pavement
should be suitably patched.

7.5 Laboratory Testing


7.5.1 Soil sample shall be tested using the relevant American Society for testing Materials
(ASTM) procedures for particle size analysis.
7.5.2 Rock samples shall be analyzed for percentage core recovery and rock quality
designation (RQD)
7.5.3 Intact core sample shall be subjected to confined compression test.

HEALTH, SAFETY AND ENVIRONMENT (HSE)

a. Risk Assessment
Based on this method statement, the activity will be risk assessed and documented
using the Job Hazard Analysis (JHA). A Toolbox Talk shall be disseminated to all
workers involved in the project before commencing any the work activity. Through
JHA, all personnel shall be more aware of the potential risks at the workplace
associated with their work activities and the precaution which must be taken to
eliminate, reduce or adequately control such risks, to prevent injury to personnel
or damage to property and the environment. An understanding and awareness
of potential risks and the control measures will prevent the accident.
b. Protective and Safety Equipment
Basic Personal Protective Equipment (PPE) as per Now Builders procedure are as
follows: Safety Shoes, Had Hat, Safety Glasses (Over The Glass) when applicable,
Steel Toed safety boots and the Company uniform will be issued to all workers
involved in the activity as well as the required PPE for special activities that may
require the use of such as face shield, working gloves and ear plugs. Other special
PPE may be prescribed as risk assessment. Hi-visibility vest with reflector strips shall
also be issued to the workers to ensure greater visibility.

c. Health and Safety General Requirements


All HSE regulatory, contractual and project requirement will be controlled at all
times shall commence or continued during High winds, heavy fog, and adverse
weather condition or when vision is impaired by darkness.
• Watchmen must be equipped with whistles during the activity. It will be the
warning signal to all workers near to heavy equipment when they will be in
unsafe situation.
• Tag lines, lifting belt/sling, and shackles are inspected by safety officer, ready
on site and in good working condition.

• First Aider will always be available at site to provide health monitoring to all
workers.
• As for COVID-19, health declaration form must be given and fill up by the
workers before activity started (Checking of Body Temperature, Wearing of
Face Masks / Dust Masks properly and all the time).
• Maintain Physical Distancing if the job doesn’t require any assistance from
others.
• Frequent hand washing is advised before and after every activities.
• Adequate First Aid Kit, Face Masks, Alcohol, Potable Eye Wash, Blood Pressure
apparatus will be made available at site.
• Hot Line / Contact Numbers for emergency work related incidents will be
posted on site. As well as the reference number of the nearest Hospital for
further medical treatments.
• Mess hall and rest areas should be available on site, including drinking and
potable water supply for all workers.

d. Health and Welfare Arrangement


• Safety Nurse/First Aider will always be available at site to provide health
monitoring to all workers.
• Adequate First Aid Kit, Face Masks / Dust Masks, Hand Sanitizer/Alcohol,
Potable Eye wash, Blood Pressure apparatus will be made available at site.
• Heat Stress Management (Temporary resting facility is provided together with
the Potable Drinking Station).
• Hot Line / Contact Numbers for emergency work related incidents will be
posted on site. As well as the reference number of the nearest Hospital for
further medical treatments.
• Mess hall and rest areas should be available on site, including drinking and
potable water supply for all workers.

e. Rainy Season
Workers’ safety should be the number one concern in all types of construction and
projects, regardless of the weather condition. But during the rainy season, the risk
of slip and fall is far worse, as rain can make roofs, walkways, roads and floors
slippery.
• Appropriate Rain Gear with high-visibility vest that includes both pants and
coat must be provided to the worker together with the Slip Resistant boots prior
to start work.

• Conduct health topic during toolbox meeting about the common diseases
that anyone may acquire during rainy season.
• Keep yourself updated to the weather from time to time.
• Equipment that requires electricity to run should be placed and used indoors
and unplug it, if not needed.
• Proper Storage & care of construction materials, equipment and machinery.
• Outdoor activities are prohibited during typhoon (heavy rainfall, strong winds).
• During thunderstorms, be extra cautious of exposed steel structures are high
conductors of electricity.
• Maintain surface of embankment or subgrade to prevent ponding. Take
precautions for floods in flood prone area.

f. Fire Precautions
• SLTEC premises is a Non-Smoking area that’s why any object that may cause
fire is strictly prohibited.
• Portable fire extinguisher will be assigned in all heavy equipment, Generator
Set, Welding Machines and light vehicles entering the sites, especially in fuel
tanks areas.
• Fire watcher should always be ready when it comes to do hot works like
welding.
• Fire Extinguisher Familiarization / Training will be conducted to all workers on
site.
• Fuel Truck and fuel containments will be isolated away from any combustible
materials. Warning Signs will be installed in these areas

g. HSE Information to Personal


• All workers involved in the activity must go through the proper Safety
Orientation on how to work safely on site to ensure total Safety Compliance.
• As for COVID-19, health declaration form must be given and fill up by the
workers before activity started (Checking of Body Temperature, Wearing of
Face Masks / Dust Masks properly and all the time).
• Physical Distancing must be maintain if the job doesn’t require any assistance
from others.
• Frequent hand washing is advised before and after every activities.
• As stated in method statement, Job Hazard Analysis (JHA) shall be prepared
for the activity and well discussed to all workers for better understanding the

• safety practices required. This will also help avoid any accidents / incidents
that may take place during on-going activities.
• Toolbox talk shall be conducted by the supervisor in charge with the help of
the HSE personnel prior to start each working day of the project on site

h. Emergency Procedures
• Assembly points shall be established and stated on toolbox talks to familiarize
workers the proper way of evacuation in case of any unexpected incidents that
may occur during working hours. Communication system shall be made
available at the work site.
• An Emergency Response Team will be formed within the group to sustain any
controllable incidents until medical assistance arrives.
• The ERT will be responsible for managing and directing the activities of the workers
who will be involved in emergency response and recovery in the workplace.
• An Emergency Response Vehicle shall be made available on site at all times
during working hours. This will be utilized in transporting any injured or ill personnel
on site to the nearest hospital.
• Emergency Hotlines must be posted and visible to the work site.
• Accounting of personnel will be conducted after each work shift.

i. Environmental Concerns:
• Spillage of fuel on the ground should be avoided at all times.
• All heavy equipment / machineries must have a regular maintenance check-up
to avoid emissions that can affect the atmosphere.
• All containers on site shall be properly maintained and secured to ensure that
there will be no spillage.
• Drip trays will be provided for all vehicle /mobile equipment during repair and
maintenance whereas a possible spillage may occur.
• Discovery of hazardous materials such as unexploded bomb, pipelines shall be
reported immediately to the client and appropriate governmental agencies for
proper handling.
