COMPREHENSIVE EXAMINATIONS

INFORMED CONSENT & STUDENT UNDERTAKING

I, Galang, Marnel Jasper A. Student No. 20572953 under the degree program Bachelor of
Science in Accountancy hereby agree and understand that:

a) I attended the consultation/orientation and I am aware that participation in the

Comprehensive Examinations is a requirement in completing the Bachelor of Science in
Accountancy.

b) I will follow all policies of the university (as stated at the Student Manual & prevailing
COVID-19 Health Protocols).

c) I am fully vaccinated (first and second dose) with the Covid-19 vaccine or I have tested
negative on a COVID19 antigen test taken 2 days prior to the examinations.

d) I will always have my personal hygiene kit (alcohol, soap, extra face mask etc.)
whenever I am inside the campus.

e) I will be submitting necessary documents in order for me to join the Comprehensive

Examinations
a. Copy of Vaccination Card or Negative COVID19 Antigen result taken 2 days prior
to the examinations.
b. Signed Informed Consent and Undertaking,

I hereby undertake that I will abide by the rules and regulations of the university. I understand
that I shall be liable for any disciplinary and legal action in case of any violation of the
guidelines.
