I would like to request a leave of absence for a period of (1 week). The

reason is that after I was vaccinated, the next day I had extremely fever
and I suddenly collapsed in the bathroom, I was dizzy after what happen.

I promise to return on the appointed day of my illness, and I will pursue

the missing activities that I did not answer and passed on time.
