Shampoo on Bed
◼ Massaging the scalp

PURPOSES:

1. To wash the hair from dirt


and oil.
2. To wash scalp and stimulate
the circulation of blood in the
scalp.
3.To prevent the infestation of
lies
4. To wash the hair
after the application of
lice exterminator.
5.To increase the client’s sense
of well- being
6.To assess or monitor hair or
scalp problems
Assessment:
Determine the following:

◼ History of the following conditions


or therapies: recent
chemotherapy, hypothyroidism,
radiation of the head, unexplained
hair loss, and growth of excessive
body hair.
Assessment:

◼ Usual hair care practices and


routinely used products
Assessment:

◼ Whether wetting the hair will


make it difficult to comb.
Assess also:

◼ a. Condition of the hair and


scalp

is the hair straight, curly,


kinky, matted, tangled? Is the
scalp dry?
Assess also:

◼b. Evenness of hair growth


over the scalp, any patchy
loss of hair, hair texture,
thickness, thinness, infections
or infestations in the scalp
Assess also:

◼ c. Self-care abilities

- are there any problems in


managing hair care
Problems or Abnormalities of
Scalp and Hair:

1. Dandruff ( Seborrheic
Dermatitis) - diffuse scaling of the
scalp often accompanied by itching.
2.Alopecia - hair loss
3. Hirsutism – excessive growth of
hair in the body

4. Pediculosis infestation of lice

1. Pediculosis capitis – head lice

2. Pediculosis corporis – body lice

3. Pediculosis pubis – crab lice

4. Scabies – a contagious skin

◼ Large Pitcher (2 )
◼ Basin

◼ Pail

◼ Kelly Pad (inflated)


◼ Comb or Brush

◼ Newspaper
PROCEDURE :
1. Verify Agency policy and the
Physician’s order

Rationale : Some Agency requires an order


2.Determine the best time of
the day for the procedure .
Discuss this with the client.

Rationale:Choose a time when client is rested


and can rest after the procedure.
3.Assemble all equipment and
arrange them conveniently at
the client’s bed side .

Rationale: To save time ,effort and energy


4.Explain the procedure to the
client .

Rationale :To gain Cooperation


5. Wash hands and observe
other appropriate infection
control procedures if needed.

Rationale : To prevent the spread of


microoragnism.
6.Provide privacy by drawing
the curtains around the bed or
using the screen if necessary .

Rationale: hygiene is a personal matter.


7.Loosen the Gown of the client
8.Fold the top sheet down to the
middle and replace it with a
bath blanket
9.Cover the chest with bath
towel crosswise.

Rationale :To prevent patient from chilling


10. Remove the pillow and bring
the patient’s head diagonal
across the bed .

Rationale: To have a better access of the client’s


head .
11.Protect pillow with a rubber
sheet and cover it with a bath
towel .
Rationale: To protect the pillow from getting
wet and to have a ready access in drying the
hair after shampooing .
12.Place the pillow under the
patient’s shoulder .

Rationale : This hyper extends the neck


13.Place the inflated Kelly pad
under the patient’s head and
place the pail on the floor
supported by newspapers.

Rationale : To have a receptacle for the water .


14.Comb the hair carefully

Rationale : To remove tangles.


15.Place the damp face towel or
wash cloth over the face
covering the eyes

Rationale: To protect patient’s eyes from soapy


water
16.Place the cotton balls on the
ear canal.

Rationale : To prevent water from entering and


collecting the ear canals
17.Moisten the hair thoroughly
with lukewarm water .

Rationale : To soften the hair .


18.Apply soap or shampoo on
hair and scalp . Massage using
the nail pads.
Rationale : Stimulate blood circulation in the
scalp .The Pads of the fingers are used so
that the fingernails will not scratch the scalp.
19. Rinse properly with warm
water .And repeat the process
until the hair is rinsed
thoroughly .
Rationale : To prevent chilling and to prevent
irritating the scalp by leaving some soap suds.
20. Rinse finally with cool water

Rationale : To refresh the client.


21. Drop the Kelly Pad

22. Squeeze the hair from


excess water.

23.Dry hair in a turban manner


24.Position the patient in
comfortable position.

Rationale : To ensure client’s comfort.


25. Document the shampoo and
any assessments
