Ambedkar Nagar, Ghatkopar, Mumbai – 400075. do hereby solemnly affirm

and declare as under :-

2. That the date of birth of the deponent is 12/07/1982 .

3. That the date of birth of the deponent is wrongly mentioned in the

School Living record as 12/07/1983.

4. That the deponent wants to get corrected his date of birth in the

School Leaving Certificate as 12/07/1982 instead of 12/07/1983.

I am executing this Affidavit to be produced before the concerned

authorities showing the above fact of my past and present Birth Date and this

Affidavit is made as per their requirements.
