I, AJAY BALAJI KAMBLE, an adult, Indian

Inhabitant of Mumbai, residing at room situated at Belapur
Road, Sector-10, Kopara Gaon, Kharghar, Raigadh, Kharghar
– 410210. do hereby state and declare on solemn affirmation
as under :

I say that my true and correct name is AJAY

BALAJI KAMBLE and the same is mentioned in my School
Leaving Certificate, my name is shown recorded as AJAY
RAJESH KODVILKAR.
 I say that both the names i.e. AJAY BALAJI
KAMBLE and AJAY RAJESH KODVILKAR are the names of
one and the same person i.e. myself and my present name is
AJAY BALAJI KAMBLE.

I am executing this Affidavit to be produced

before the concerned authorities showing the above fact of my
past and present name and this Affidavit is made as per their
requirements.
