Doctor: (General observation: check for dehydration look for sunken eyes)
Doctor: (confirm) ingani najud na eyang mata sauna pa nga wla pa cya nagkalibanga?
Mother: nag ilawm na cya pag kalibanga nia
Doctor: (get a cup of water or spoon) ako testingan kung gi uhaw cya ipa inom dw nia maam
Mother:( lets him drink but doesnt drink bec lethargic) wla mn cya ni inom doc
Doctor: palihug ipahigda cya sa examine table ipa lie flat eya likod nia eya kamot naa sa kilid nia guniti
lng cya aron di mo lihok.
Mother: okay doc no problem 
Doctor: (pinch the abdomen) skin goes back slowly
Doctor records the results.. 
Doctor: nurse sitti palihug ko kuha sa chart, thank you!
Nurse: here doc, (handle the chart to doctor)
Doctor: naa cyay acute diarrhea kay 3 days pa og severe dehydration tungod kay lethargic, sunken eyes,
skin goes back slowly when pinch.

Mother: ang pangalan sakong baby kay Gail Alonso, 4 months old, baby girl, 38 weeks ni sia sukad
gipanganak nako. UCCP among religion. Naa mi gapuyo karon sa Talamban, Cebu City. Natawo ni sia
atong September 1, 2021 dinhy sa Vicente Sotto.
