I, Lamberto D. Ronquillo, of legal age, married, Filipino and a resident Dau, Mabalacat
City after having been duly sworn according to law, hereby despose and state:

That I am the absolute and registered owner of “L.D.R. Bike Shop” located at Mac
Arthur Hiway, Dau Mabalacat City;

That sometime on January 10, 2022 I closed the said business establishment because I
cannot manage it properly due to my age that’s why I transfer it to my wife.

That this affidavit is being executed to attest to the truth of all the foregoing facts and to
inform all offices of government regarding its closure.
