AFFIDAVIT OF LOSS

I, HAZEL JEAN C. ALIÑABON, of legal age, married and a resident of

Tumaga, Zamboanga City, Philippines, after having been duly sworn to in accordance
with law, depose and say, that:

1. I am an employee of Banco de Oro, Rizal Branch, Zamboanga City and as

such I was issued an Identification Card Numbered 0000261378;

2. I usually keep said Identification Card inside my purse whenever I am not

within the office premises. However, on March 27, 2021, I discovered that
the said Identification Card was no longer inside my purse and was lost;

3. I exerted diligent efforts to look for said BDO ID but the same proved futile.
For all intents and purposes, I am now declaring the my BDO ID Numbered
0000261378 lost and beyond recovery.

4. I execute this Affidavit to attest to the truth of the foregoing facts based on
personal knowledge for whatever legal purpose it may serve.
