I, the undersigned, hereby declare and affirm that I am the parent/guardian

I hereby give my full consent to send my ward to the school campus as a
volunteer for Summer Camp Activities. I hereby declare and affirm that my
child is physically fit to take part in the Camp’s activities and my child has
no known illness or adverse medical condition that would render him or her
unfit to participate therein. We know that the teachers and administrative
staff of the school take utmost care of our ward. In spite of that, if any
untoward incident occurs, we undertake not to hold the school or
management responsible for it whatsoever.
