ACTIVITY 2

MY CONTRACT FOR CHANGE

Direction: Fill out the form with the necessary data.

DATE: September 11, 2020

PERSONAL GOAL/S: Become physically fit and physically healthy. Make sure that my lungs
is always in good condition to avoid experience having asthma attack again. And do a lot of
exercise, if possible facial exercise if there is like that to help me heal my Bell’s palsy, I feel
nothing on my right face and I cannot move it. (I have an asthma since I was 5 years old and
started to have Bell’s palsy 2 years ago)

MOTIVATING FACTOR/S: My goals in life, my dreams and the plans of my family for
myself. Because if I could not cure my illness, it will be hard for me to focus on my studies if I
keep on having asthma.

CHANGE/S I PROMISE TO MAKE TO REACH MY GOAL: I promise to give attention to


my health and spend more time to exercise, do some recreational activities, and avoid things that
is not helpful in achieving my healthy lifestyle goal. Always eat healthy foods, avoid fatty one,
and most specially the one that I cannot do right now, sleep on time I promise to really do this.

PLAN FOR MAKING THIS CHANGE/S: Since I love dancing, (me and my mom having
Zumba every weekend) I will make it is a tool to improve my physical figure and become
physically fit, not only because it help me to become sexy but because dancing is a great help in
having a good cardiorespiratory endurance. This means that the more oxygen is efficiently and
effectively transported using the body’s transport system which are the lungs, heart and blood
vessel. I will continue my Zumba, but I will make it every day since there is no face-to-face
classes, I will spend at least 30 minutes of dancing every day.

START DATE: September 14, 2020 (Monday)

ASSESSMENT PLAN: I will monitor myself, if my asthma does not visit for about 2-3 weeks
then my plans are effective.

IF I NEED HELP: I will ask for my mother since she is also a fond of dancing. I will also read
books about how to become physically fit and browse the internet.
TARGET DATE FOR REACHING THE GOAL: If possible within this year, if successful, I
will continue it for the following more years.

REWARD FOR ACHIEVING THE GOAL: I will buy myself new clothes or new shoes that I
can use for my Zumba routine.

PENALTY FOR FAILING: I will give my favorite shoes and clothes to my sister and cousin.
