AFFIDAVIT OF SUPPORT

I, SHANE TIFFANY G. BOLAÑOS, of legal age, Filipino and a resident of 75 Victory

Avenue, Tatalon, Quezon City, after duly sworn to, in accordance with law, hereby depose and
say:

1. That I am the sister of JERICO MHAR G. BOLAÑOS, born on September 11, 1999 in
Quezon City;

2. That he planning to study for the Admission of College of Medicine in Pamantasan ng

Lungsod ng Maynila;

3. That I give my support to this admission and freely by declaring under oath;

4. That I also supporting his the expenses including tuition fee, miscellaneous fee and
other expenses to be incurred during his study;

5. That I have financial capacity to finance his expenses while and during his study;

6. That I am executing this affidavit to attest as to the truth of the foregoing statements in
order to comply with the requirements in connection with the visa application.
