for Supervendor Project of Globe Telecom;

2. We assigned to secure permits from local government or national government

agencies for the construction, operation and maintenance of telecommunication
facilities and equipment of Globe Telecom, Inc.(Globe) located at Brgy. Masiag,
Bagumbayan, Sultan Kudarat.

3. Relative to the declaration of RTB, CPG East Brgy. Bool tagbilaran City at
Undertake submission of CAAP Clearance.

4. On behalf of EFB Geostrukt, Inc, I shall comply with the completion of

requirements necessary for Ready to Build.

5. I am executing this affidavit to attest to the truth of the foregoing facts and for
any legal purpose this may serve.
