JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MARIA TERESA M. LALIC and JOCELYN LALIC ALMEDILLA both

Filipinos of legal ages, married and residents of Purok 5, Concepcion, Montevista,
Davao de Oro and Purok 3, Concepcion, Montevista, Davao de Oro, Philippines
respectively having been duly sworn to in accordance with law do hereby depose and
state THAT:

1.) We both personally know the person of MARICEL BANTILES BANGOY being
acquainted with her for a long period of time;

2.) In the PHILHEALTH record of MARICEL BANTILES BANGOY, the same was
inadvertently entered as “MRICEL BANTILES BANGOY” instead of
“MARICEL BANTILES BANGOY” and that her date of birth is “September 29,
1984” instead of “September 30, 1986”;

3.) We declare, that “MARICEL BANTILES BANGOY” and “MRICEL BANTILES

BANGOY” are one and the same person;

4.) We declare, that the real date of birth of MARICEL BANTILES BANGOY as
appearing in her marriage contract is September 30, 1986 and not September
29, 1984;

5.) We are not related to MARICEL BANTILES BANGOY, by consanguinity nor by

affinity;

6.) We are voluntarily executing this Joint Affidavit to attest to the truth and veracity
of the foregoing statements and to apprise the proper authorities concerned on
the matter for their information, guidance and for whatever legal purpose/s this
may serve best.
