eligible or qualified for the renewal of my notarial commission which the Honorable Regional
Trial Court of Cebu City granted me on December 26, 2021. My current notarial commission is
originally set to expire this December 31, 2021.

Meanwhile, I have completely submitted to the Notarial Section of the Honorable Court my
notarial reports together with the certified true copy of the pages of my notarial book, and the
duplicate original of the documents I have notarized from November 01, 2019 to October 31,
