INCIDENT REPORT

Name of Employee: Kenneth Bryan Gantalao Position: Customer Service Representative

Description of the Incident: Agent has breached the company agreement for being employed with
another BPO industry.

On November 1, 2021, management posted and convened a huddle to announce that we have a
new memo to sign, one of which is the number 4 Memo, which states that "As indicated in our
Employment Contracts, we are not allowed to engage in any other business WITHIN our employment
with BPO Made Easy." This means that we are not permitted to be associated with any other contract
employment.

The agent has breached the company agreement for being employed with another BPO
industry. For more information, please see the image below.

1.In the first image, the agent was on a Payment Verification call when he received a
SMILZ call from Inbound One, which he did not answer, indicating that he had a dual login with his
Dialpad as when the agent received a call from PVT, it would show Payment Verification and not
SMILZ unless the agent was logged to Inbound One logins.

2. In the second image, the agent opens an unrelated app (LaunchQ) and a group chat called "**TPG -
TEAM DAN**."
