JOINT AFFIDAVIT

We, RALPH JIREH P. OBNIALA and LOVELY P. CARPIO, both of legal ages,
Filipino citizens, and residents of Buag, Bambang, Nueva Vizcaya, and of Pingkian,
Kayapa, Nueva Vizcaya, after having been duly sworn to in accordance with law, do
hereby depose and state:

1. That we will be receiving the Sacrament of Matrimony, to be officiated by REV.

ORLANDO R. BALUNSAT of HARVESTER BIBLE BAPTIST CHURCH IN THE
PHILIPPINES INC. with Registry No. 2022-29WG2MQULT-2024, on April 9, 2022;

2. That we are requesting REV. ORLANDO R. BALUNSAT to solemnize our wedding

at Casa de Española, Barangay Tucanon, Aritao, Nueva Vizcaya;

3. That we are executing this affidavit as a requirement to our upcoming wedding

which will be held at Tucanon, Aritao, Nueva Vizcaya, on April 9, 2022;

4. That we likewise execute the same with our own free will, act and deed and in
support to legalize our wedding.
