OBJECTIVE

• This course is intended to


provide basic knowledge of the
Lock out/Tag out (LOTO)
Program for all Affected
Employees.

LOTO Roles - Affected Employee
• A person whose job requires him/her
to work on a system or equipment and
a LOTO is required to protect them
from the hazardous release of energy,
or whose job requires him/her to work
in an area where such activities are
being performed (all non-Authorized
Tagging Authorities on the project
site).
Upon completion of training, Affected
Employees will receive a white hard hat
sticker to indicate their level of training.

Purpose of LOTO Program

• LOTO is a procedure used to control sources of hazardous energy


• Example of hazardous energy include:
• Electrical
• Pneumatic
• Hydraulic
• Potential
• Thermal
• Mechanical

Any type of energy that can harm a person is a hazardous energy

HAZARDS
LOTO PRACTICE

• Energy isolating devices, locking devices,


locks, and tags are used to safely isolate
sources of hazardous energy
• Examples of isolation devices include:
• Circuit breakers
• Disconnect switch
• Manual isolation valves
• Blank flange, slip blind, or other
physical block inserted into piping
• Physical separation (e.g. lifted leads,
removed pipe spools, misaligned pipe)

LOTO PROCEDURE

• All LOTOs must be initiated by an


Authorized Tagging Authority (ATA)
• The ATA is the primary holder of the
LOTO, other holders (including
Affected Employees) may sign on and,
if applicable, hang their locks

Personal locks placed on energy isolating


devices without a LOTO currently in place are
not allowed.

Do Not Operate (DNO) Tags

• When an energy isolating device has been


isolated as part of a LOTO a red Do Not
Operate (DNO) tag will be hung on the
device
• Each lock and locking device must be
accompanied by a DNO tag

PERMIT TO WORK (PTW) Procedure

• If at any time Construction needs to


perform activities on a system that has
been turned over to Commissioning a
Permit to Work (PTW) must be obtained
first
• Construction must request a permit from
Commissioning prior to activity starting
• Commissioning will identify hazards and
necessary LOTOs to be performed to keep
Construction employees safe

Interaction Between Construction
and Commissioning

• Upon completion of construction work,


systems are turned over from Construction
to Commissioning
• When a system is turned over to
Commissioning yellow System Under Test
(SUT) tags will be hung on the components
of that system

Becoming a HOLDER OF A LOTO
• To become a HOLDER OF A LOTO you should complete the
following steps:

1. If the LOTO does not already exist, notify your foreman and
an ATA about the need for the LOTO. The ATA can initiate the
process.
2. Once the LOTO has been created, review the isolation log
and verify that all hazardous energy sources have been safely
isolated.
3. Sign on to the LOTO Holder sheet located in the LOTO
Office.
4. Hang your personal lock on the lock box located in the
LOTO Office.

• If using a remote lockbox:


• If your foreman has signed on as a Holder and placed
his lock on the box in the LOTO Office, hang your
personal lock on the remote lock box holding the
foreman’s key.

NOTE: All Foreman and Personal locks must be removed


at the end of every shift.

LOTO EMERGENCY RELEASE PROCEDURE

Under extraordinary circumstances, it may be necessary to


remove a Lock or Do Not Operate (DNO) tag that has been
affixed by someone else or without the proper signoff of a
Holder.

ONLY the Tagging Manager may authorize the removal of a Lock


or DNO tag, and only in accordance with the following
stipulations:

1. The Authorized Tagging Authority who


is responsible for the Lock or DNO tag or
a Holder of the LOTO is not present at
the site, and all reasonable efforts have
been taken to contact the responsible
employee.

• Proper hazardous energy control is


required to keep everyone on the job site
safe
• Any violation of the LOTO procedure will
result in removal from the project,
including, but not limited to:
1. Working exposed to hazardous energy
without a LOTO
2. Working on a system turned over to
commissioning without appropriate PTW
3. Removing a DNO or SUT tag
4. Operating any Energy Isolation Device
under a LOTO

This is just a brief description of the


Lock Out Tag Out Procedure.

It is imperative that there is a complete


UNDERSTANDING of the dangers and/or hazards
working on or near equipment that have some
form of energy either stored or applied to it.
