Using the following criteria for evaluating questionnaire developed by Good and Scate,
please evaluate the attached questionnaire (PARENT SURVEY) for the proposed study (1 being
poor and 5 being excellent)

Criteria for Validity

1. The questionnaire is short enough such that the respondents will not
drain much of his precious time.

2. The questionnaire has a face appeal such that the respondents will be
inclined to accomplish it fully.

3. The questionnaire can obtain some depth to responses and avoid

superficial answer or information.

4. The items and their alternative responses are too suggestive or to too
stimulating.

5. The questionnaire can elicit responses which are definite but are
mechanically forced.

6. The items are stated in such that response will not be embarrassing
the person concerned.

7. Item are framed in such manner as to allay suspicion on the part of

the respondent concerning hidden purposes in the questionnaire.

8. The questionnaire is not too narrow, not too restrictive in its scope or
philosophy

9. The responses to the questionnaire when takes as a whole could

answer the basic purposes for which the questionnaire is designed and
therefore are considered valid.
