The following suggested activities may be included as part of the school’s activity: a. Parents-Teacher-Adviser Conference and Feedback- Giving; b. Showcase of Learners’ Outputs and Performances; ©. State of the School Address and Recognition Program; and d. Distribution of Learners’ Progress Report Cards. 3. "The schools may choose the appropriate modality such as on-site, off-site or depending on the status of COVID 19 cases in the locality and provided that health protocols are complied with, 4. Monitoring teams are organized to ensure the smooth and meaningful conduct of the activities, (See Enclosure 1.) Likewise, the District Supervisors shall also monitor and supervise the activity in the schools within his or her district. 5. __ The schools shall submit accomplishment reports using the enclosed format to be consolidated by the District Office and submitted to the Division Office (Attn: CID) on or before December 6, 2021 through depedsamar.cid@deped.gov.ph and eloisa.zartigaadeped,gov.ph, (Please see Enclosure 2.) 6. The Portfolio Day Monitoring and Evaluation Tool shall be used by the Division Monitoring Teams during the conduct of the activity, The accomplished M and E Tools shall be submitted to the CID Office within one week after the conduct of the activity. (Please see Enclosure 3) 7.
