Implementation of the guidelines is essential to allow a means of safe access to work fronts using scaffolding that is assembled, inspected and maintained in a safe and accident free environment. Allso it describes the process of site scaffolding installation works in the step by step. . SCOPE ‘The document to detail the procurement, installation and Safety Control requirements for scaffolding works in accordance with Nghi Son Refinery project Regulations. All scaffolds / temporary work platforms must be constructed in conformance to NSRP HSSE. manual and must contain as minimum, safety features herein prescribed. 3. DEFINITION Employer: ‘Nghi Son Refinery and Petrochemical Limited Liability Company. Contractor: A joint Venture formed by JGC Corporation, Chiyoda Corporation, GS Engineering & Construction Corporation, SK Engineering & Construction Co.,Ltd, Technip France and Technip Geoproduction (m) Sdn. Bhd, Subcontractor: Petrolimex Construction JSC-1 (PCC1). 4, REFERENCE, All scaffolding shall be constructed base on acceptable engineering codes and practices such as GOST 24258 and British standard BS55973 (Code and practice for access and working scaffolding and special scaffolding structure in steel). All scaffolding material shall be sound condition and confirm to GOST 27321-81 British standard BS1139/BSEN74 or equivalent unless otherwise written approved is obtained from the contractor. Is the event of any conflict between the requirement of this procedure and the above standard, the ‘more stringent requirement shall govern. All of loss prevention activities in construction field will be practically operated under his responsibilty. Construction Superintendents! Supervisors will atively administer their department loss prevention manuals; provide supervision and training required to ensure safe practice employees; and conduct periodic inspection of their construction site, equipment and work areas to detect and correct hazardous condition and unsafe acts ‘The HSSE Officer/ Supervisors report directly to Construction Manager. He is responsible for the establishment of an effective HSSE manual, which is based upon Contractor HSSE policy, meets the CLIENT'S contractual requirements ans the HSSE control procedures, 5.2 Scaffolding Works Statement ‘All workers shall received induetion training and training work at height training from HSSE_ Department. All scaffolders must hold scaffolder card in compliance with Project Standards. All. workers shall wear proper PPE et all times while working on site. Only nominated person is permitted to erect, inspect, use, move, alter andlor dismantle scaffold. [All scaffolding works shall be caried out in accordance with British Standards and HSE, requirements. ‘Al scaffolding materials used on site inspected and will be appropriate free from defect and comply with BS. Lifting equipment shall be suitable for the task and shall have been tested and examined by HSSE. All deliveries where powered lifting is used shall require proof of operator competence and that the lifting equipment has a current certificate of test and through examination. ‘The area under lifting works shall be barricaded with sign board before lifting activities and unauthorized person shall not be permitted to enter this area. All lifting activities shall be conducted under controlled by supervisor. Each scaffold structure, including tower scaffold, shall be registered and be physically marked to record regularly. Scaffolding tags and sign boards shall be used to display the scaffold status, 53 Scaffolding Installation and Dismantling. ‘Where practicable, the erection and dismantling of scaffolding is to be carried out using @ sequential ‘method that provides fall prevention for team members. As a minimum the first 4 lifts (8m from ground/platform level) of a scaffold being erected and the last4 lifts of a scaffold being dismantled are to be completed using a sequential erection/ dismantling method. ‘Additional weights imposed by adopting a sequential erection/dismantling method are to be considered during the scaffold planning stage. Where the sequential erection/dismantling method is impractical, a risk analysis is to be conducted to identify the most practical fall prevention method. Asa last resort fall arrest systems (full body hardness, lanyard or inertia reel and anchor point) are to be provided where no other protection or prevention measure is practical. Fall prevention is to be in accordance with the working at height procedure, Doc No.: $-000-1654-0510V. 54 General Requirements 5.4.1 Working Places, General Falls: Where men, tools and materials could fall 1.8, meters or more, a guardrail and toe boards shall be provided. Stairs shall have all treads prop. 5.4.2 Requirements Common to all Seaffoldings All scaffold structures shall be erected with approved metal components. Sometimes timber scaffolds may be able to use. Scaffolds shall be stored to prevent damage and to permit easy access for use. All scaffolds or temporary work platform shall be designed and erected to support at least four times the maximum intended load or weight they are expected to support when in use. Only competent, well-trained and dedicated scaffolding builder! erector shall be allowed to erect, modify and dismantle supervise by a competent scaffolding foreman. No planks, tubes and couplers with sign of damage should be used on scaffoldings. Defective scaffolding material shall be readily isolated from the work area/ disposed from the site Scafflolds will be inspected by a qualified! competent scaffolding foreman/ supervisor! inspector prior to use and/ or remove. No Rev-0 INSTALLATION 5.4.3 Tie-off Policy Scaffolders and all other personnel working in an unprotected level at or above 2 meters in height (6 feet) from the ground or floor, who are exposed to potential fall, shall wear full body hamess with shock absorbing lanyard as minimum. The lanyard shall be anchored at all times to an anchorage point such as a lifeline or rigid structure or a stable part of the scaffold structure at the highest point possible and not lower than shoulder level. Anchorage point must be capable to ‘withstand at least 5,000 Ibs. of foce. Where the situation warrants, double-shock-absorbing lanyard must be employer to attain 100% tie-off'during the acture scaffold errection, dismantling of repair. ‘When so employer,the second lanyard must be properly anchored to the desired anchorage point before the first lanyard is disengaged. While determining the rate of the erection an dismantling of suspending & cantilever scaffolding, the elevation of point of suspension shall be considered as per the elevation of the actual scaffold platform. Handover tag for scaffolding structure shall be submitted and measured every 20 meters (length-wise) Example: If a scaffold is suspended from 15 m elevation from ground, the rate for 10-20 m height band shall be applied. 45.5.3 Stand Alone Type (Tower/Bird Cage/Independent) The scaffold area standalone type of scaffolding shall be calculated in Cubie Meters (m3/Cu. Erection & dismantle rates shall be applies to the total volume of scaffolding in cubic meter according to the elevation of the scaffolds. Handover tag for scaffol 1g structure shall be submitted and measured upon completion of ‘each scaffolding structure for pipe rack access tower. Example: If a tower type scaffold is erected on ground elevation with one board level, it shall be charged at the rate for 10 ~ 20 m height band. 1f.a.22 m tall tower type scaffold with one boarded level is erected at 5 m elevation from ground, scaffold the rate for 20 ~ 30 m height band shall be applied.
