Title of Assignment/Activity: Chapter 5 Test

Class (check one):

☐ Science ☐ Math ☐ Social Studies ☐English ☐ Band/Orch. ☐ Elective ☐ Extra Curricular
Type (check one):
☐ Lab ☐ Project ☐ Group Work ☐ Test/Quiz ☐ HW ☐ Essay ☐ Concert ☐ Contest ☐ Other

Short Answers:
1. In at least 2 sentences, describe what you did to complete this assignment/activity:
To complete this test I studied, used what I had learned, and double-checked. To study I looked over the
homework checks that had problems covering all sections of the chapter. I also took the practice test
which covers the main points of the chapter. After this, I used what I knew to solve the problems on the
test. These include substituting, canceling out the other side of the equation, reading a graph, and writing
linear equations. Instead of putting in the x and the y values, I put in the sum of them because that is
how Mr. Martin prevented cheating. After I was finished with the questions I double-checked them and
even caught a few mistakes that were due to not reading the question carefully enough the first time.
After double-checking, I submitted the test.

2. This assignment/activity showcases my (check one):

☐ Academic Skills ☐ Personal Management Skills ☐ Teamwork Skills
(How smart I am) (I am good at being a student and using time wisely) (I work well in groups)

3. In at least 2 sentences, explain why this assignment/activity demonstrates the skill checked above:

This test demonstrates academic skills because it shows that I know everything taught in chapter 5. I showed
understanding of solving for variables in linear equations, reading graphs, and finding the slope of lines. Last,
this test shows that I can carefully read directions and follow them.

4. Choose one of the following questions to answer about your work (check one):
☐ What makes this work one of your best?
☐ What problems did you encounter as you were working and how did you overcome them?
☐ What are some of the strengths and/or weaknesses of this work?
☐ Why should you share this work with others?
☐ How could you improve this work?
5. In at least 5 sentences, answer the question you chose above (number 4).
Some strengths of this work are the ability to go back to notes, a second try if needed, and lots of time. For this
test I answered the questions in a notebook so I had somewhere to write. If I forgot how to do a problem I was
allowed to look at my notes, which is very helpful. Another strength is that we were provided a second try if we
didn't get a full score on the first, and Mr. Martin would take the average score. The test was due at 11:59 p.m.
and there was no time limit, so it was also not stressful.
