Questions regarding the abuse: 1. Age ofthe biethmotrer when the abuse happened. 2. Who is the perpetrator? I the perpetrator is related to the birthmother, pleese speciy the relationship (Ie. cousin, uncle, father, etc). 3, Wes the abuse reperted to the birthmother’s parents? If yes, what actions did the birthmother's parencs take against the abuser? If not, reason for not reporting the abuse. 4, Was the abuse reported to the authorities? Conduct risk assessment to determine if protective custody is necessary. The social Worker should also ensure that the birhmother receives counselling or therapy in ‘der to help her recover from the trauma of the abuse she experienced. If the birthmother is a minor and s willing to file @ case in court but has no support from her family, the social worker shoulé make the formal complaint on behalf of | the minor birthmather, The socal worker should also monitor the progress of the | cease in court. The dlagnesis/indings should be Indicated In the child study report. If the birthmother was committed to a psychiatric fact, where, when and how long? Copy of the report from the psychiatric faclity should be secured. Ifthe birthmotner was treated at a mental hospital, the socal worker should assist the birthmother's nearest kin in securing copy ofthe psychiatric report and other medical records. ‘What are the possible causes of the birthmother’s condition? I the birthmother taking medication, please specify the medication (generic name of ‘the medicine and dosage) and how long the birthmather been on medication. If there are improvenents noted In the birthmother’s behavior/s as a result of the medication/s, please specify in the report. Does the birthmather’s condition exist in the family?
