1. Application Form for Retirement of Business (Should be signed by the President /


Chairman / Owner / Authorized Representative authorized in writing or Special Power of
Attorney)
2. Barangay Certification of Closure with effectivity
3. Last Issued Mayor’s Permit for Business and Business Plate
4. Sworn Statement of Gross Sales/Receipts / B.I.R. Tax Returns (Quarterly/Annually) of
unpaid taxes (with breakdown if B.I.R. is consolidated or with branches)
5. If Corporation or Cooperative – Resolution approved by the Board of Directors
If Partnership – Partnership Agreement
