LEOPOLD'S MANEUVER

Good morning ma'am! Today I'll demonstrate about Lopold's Maneuver. This is common
and systematic way to determine the position of a fetus inside the womb.

1. Greet and identify the patient. Explain the procedure. Good morning ma'am, I'm
Hannah
Landingin and I wil lbe your nurse for today. I will perform Leopold's Maneuver to
estimate
your baby size, parts and determine the presentation and position. Would you like
to go in the
bathroom before we start? It might be uncomfortable for your later during the
palpation.

2. Wash hands to prevent the transfer of microorganisms.

3. Place the client into a flat lying position or in supine position with knee
slightly flexed.

4. Assume that the it has clothes and exposed only the part needed in the procedure
to
provide client's privacy.

5. Using warm hands, palpate the client's abdomen. Cold hands may stimulate uterine

contraction. Use the palm not the fingertips because it can tickle the client.

6. The first Maneuver is the Fundal Grip. Stand beside the patient and lay two
hands
above the abdomen just below the xiphoid process. This maneuver determines the
presentation
of the baby whether cephalic or breech. Palpate with the fingertips of both
hands to define the fetal parts present. If it is hard and round mass, then it is
the head.
If it is soft and irregular, then it is the buttocks.

7. The second Maneuver is the Umbilical Grip. Place the hands on each side of the
abdomen. This
maneuver locates the back of the fetus and the extremities. The right hand will
apply gently
pressure on the abdomen and the left hand will move in circular motion, palpating
if
there is mass present. Repeat the palpation on the other side of abdomen. If
numerous
nodular massess are palpated, then this are the fetal extremities. If one palpates
smooth,
flat and hard surface then, it is the fetal back.

8. The Third maneuver is the pawlick's grip. This maneuver determine if the
presenting and
fetal part are engaged or not. Gently grasp the abdomen using the thumb and the
index finger
of the dominant hand. Place it just above the symphysis pubis and bring together
the fingers.
If it is not movable, then it is engaged. If it is movable, then it is not engaged.

9. The fourth Maneuver is the Pelvic Grip. This maneuver determines the assessment
of fetal
weight and amniotic fluid. Face the patient's feet. Place the both hands on the
lower
abdomen above the inguinal ligaments to determine the fetal attitude sliding
towards the
symphysis pubis. If the head is present, you can feel the cephalic prominence. If
the fetus
is engaged, then it can't feel the cephalic prominence.

10. Locate for the fundic height. Using the tape measure with the centimeres. In
the
symphysis pubis until the fundus only.

11. Assist the client into comfortable position to prevent the orthostatic
hypotension.

12. Wash hands.

13. Document the procedure to provide an accurate data for the patient's care.
