1) Nomination from company to “Chief of Training –EWA” , stating that you are working as Engineer/
Technician / Wireman / Plumber / Cable Jointer Signed by Owner or Director with company stamp .
2) Photocopy of qualification Certificate Signed by Owner or Director with company stamp.
3) CPR Smartcard latest printout with clear photo
4) Photo 30 x 35 with white background -- 1No.
4a) Write CPR No & Name backside of photo

5) Test fee bill Original -- Bill No…………………………………………………. Date :- / / Pending

6) ONLY for Renewal / Transfer Original License , Smart Card Print & photo.
7) Engineers: attach photocopy of CRPEP License.
8) Technician: bring original qualification certificate.
9) Plumber qualification certificate not required.
10) In case of not being able to attend the test on the assigned date, the Licensing Unit must be informed in
writing at least one week before the test date. Failure to do so will result in loss of fees, and the re-
application for a new test date upon re-payment of the appropriate fee.
11) “Please note that it is the responsibility of the candidate to ensure compliance with all the stated
requirements, and the submission of all relevant documentation.
Failure to do so may result in delays as well as financial loss in addition to the possible removal of any
licence or qualification associated with the application.”
