You are the Registered Nurse of Westend After Hours General Practice Clinic
(WAHGPC). Mrs Delma Eggleston presented to the clinic complaining of
shortness of breath and a persistent cough.

Social History:
Lives with her husband
Husband is a non-smoker
Likes gardening
Doesn’t drink / smoke
Sometimes takes betel quid
Family History: None to report

Medical History:
Type 2 diabetes mellitus (21/9/2002)
Hypertension (15/7/2007)
Stomach ulcers (23/8/2008)
Ankle injury (11/6/2009)
COPD (19/7/2012)

Medications:
Metformin 500mg t.d.s (oral hypoglycaemic)
Ramipril 5mg daily - for hypertension

Presenting Symptoms:
Intense coughing
Pain in the chest, shoulder and back
Shortness of breath
Change in voice
Harsh sounds with each breath
Change in color and volume of sputum

Diagnosis
Chest X-ray - not clear
CT-Scan - positive
Stage 2A (lung cancer)
The tumor is 5.5 cm
Cancer cells spread across lymph nodes

Plan: Refer to Dr Andrew Kretser for further treatment

Writing Task:
Using the information in the case notes, write a letter of referral for further
treatment to Pulmonologist, Dr Andrew Kretser, at City Hospital, 27 High Rd,
Newtown, outlining the details of the patient.
