SELF DECLARATION

I, Sri Swapan Chanda Sarkar, S/o Nibaran Sarkar, a permanent resident of village and P.O. Debnagar,
Jalpaiguri, I intend to declare that on 9th November, 2021 at about 7 :30 p.m., when I was returning
from Fulbari to Jalpaiguri on my bike ,a car hit me from behind and it fled by. As a result, I lost
control of my bike and had fallen down on the left side of the road. I was wearing a helmet ,
therefore my head was not hurt but by that time my left arm was completelt numb. At the place of
the incident , the passer by vehicles stopped by and one of them took me in that condition to the
Jalpaiguri Super Speciality Hospital and I admitted thereby. But, my treatment was not initiated full
fledgedly and due to that reason , I shifted myself at Neotia Getwell Nursing Home, Matigara and got
admitted there.
