of Block 26 Lot 26, Narra Village, Neo Calapan Subdivision, M. Roxas Drive, Brgy.
Sto. Niño, Calapan City, Oriental Mindoro, after having been duly sworn in
accordance with law, depose and state that:

1. I am the complainant in the case entitled “JAMESY I. REYES ETAL., vs

NEO-CALAPAN HOMEOWNERS ASSOCIATION”, docketed as HLURB Case No.
RIV-070517-0889 before the Housing and Land Use Regulatory Board;

2. I have caused the preparation of the same;

3. I have read the counter appeal memorandum and the allegations therein
are true and correct based on my personal knowledge and authentic records;

4. I certify that I have not commenced any action or filed any claim involving
the same issues before any other court, tribunal or quasi-judicial agency;

5. To the best of my knowledge, there is no such pending action or claim;

6. If I should learn that a similar action or claim has been filed or is pending,
I shall report such fact within five (5) days from the discovery to this Honorable
Office.
