AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, REGINA G. MALIWAT, of legal age, Filipino, and presently residing at Bldg 19
Unit 111 La Mesa, Ugong, Valenzuela City and ISIDRO GONZALES MEJARES, of legal age,
Filipino, and presently residing at Blk3 Lot 17 Rockville Compound, Caloocan City, after having
been duly sworn to in accordance with law, hereby depose and state that:

1. We are friends and neighbors of ROMUALDO GONZALES ABING since 1980

or for more than forty (40) years;

2. We know for a fact that his true and correct first name is ROMUALDO
GONZALES ABING and his birth date is August 11, 1961;

3. We were informed by ROMUALDO GONZALES ABING that in his Social

Security System (SSS) record, his first name was mistakenly indicated as
“LORENZO” instead of “ROMUALDO” and his birth date as “AUGUST 10,
1961” instead of “AUGUST 11, 1961”;

4. ROMUALDO GONZALES ABING who was born on August 11, 1961 as

indicated in his Certificate of Live Birth and ROMUALDO GONZALES ABING
who was born on August 10, 1961 as shown in his SSS record are one and the
same person;

5. We are executing this affidavit in order to publicly confirmed of the foregoing

facts and to explain the discrepancy in the SSS record of ROMUALDO
GONZALES ABING regarding his first name and date of birth and for whatever
legal purpose this may serve.
