Patient: “Hello nurse, good morning too. I want to check
my illness.”
Nurse: “Yes, come in please. I’m sorry, how can I call
you?”
Patient: “Oh, you can call me mrs Fazana.”
Nurse: “Could you give me any information about you?”
Patient: “Yes, of course.”
Nurse: “When were you born?”
Patient: “8 0ctober 1993.”
Nurse: “When were you born?”
Patient: “Jombang, East Java.”
Nurse: “What is your address?”
Patient: “I live in Bantul, Yogyakarta.”
Nurse: “What is your job?”
Patient: “I am a seller in the market.”
Nurse; “Are you married?”
Patient: “Yes”
Nurse: “Do you have any allergies?”
Patient: “No, I don’t have.”
Nurse: “What is your phone number?”

Nurse: “What happened to you?”
Patient: “I feel pain in my leg.”
Nurse: “Can you describe the pain?”
Patient: “My leg was throbbing and I have wound in my
heel but long time not dry.”
Nurse: “Where does it hurts? Show me where it hurts.”
Patient: “In my toes until my ankle.”
Nurse: “Does anything make them worse?”
Patient: “When I ate sweet food.”
Nurse: “How long do they last?”
Patient: “Since two months ago.”
Nurse: “what was your last blood preassure check?”
Patient: “My blood pressure is 140/85 Mm/hg.”
Nurse: “What is your weight?”
Patient: “I have lost 5 kilos in the last month.”
Nurse: “Are you taking medication at the moment?”
Patient: “No, but I always drink the diabetasol milk?”
Nurse: “How many times did you drink it?”
Patient: “I drink it twice a day.”
Nurse: “Are your close relative alive?”
Patient:”No, I had an elder sister. But, she died in her
thirties.”
Nurse: “Do you know the cause of death?”
Patient: “Like my grandfather, a diabetes.”
Nurse: “Do you have any children?”
Patient: “Yes, a boy. He is ten years old.”
Nurse: “After this the doctor will give you a prescription
and you can buy it in the pharmacy.”
Patient: “Yes, thank you nurse Ghaida.”
Nurse: “The prescription drugs is taken regularly as the
doctor’s recommends. And eat less of that sweet stuff.”
Patient: “Okay nurse.”
Nurse: “I hope you get well soon miss.”
Patient: “Amin, thank you so much.”
