I, ARNOLD G. DABU, of legal age, Filipino Citizen, with residence at No. 15 M.

Aquino St. Sta. Cruz Lubao, Pampanga after having been duly sworn to in accordance with law
do hereby depose and state:

1.) That I am the owner of a certain motorcycle more particularly described as

follows:

Make and Type : SKYGO

Model : WIZARD 125
Chassis No. : LX8PCJ504LE002697
Engine No. : 156FMI2L1039002
Color : BLACK/BLUE
2.) That I purchased the aforementioned motorcycle on February 8, 2020 at SKYGO
Marketing Corporation;

3.) That I opt to use the above mentioned motorcycle as a service; Thus, I decided
that I need to ATTACHED a SIDE CAR;

4.) That the attachment of the side car shall also be registered with the said
motorcycle;

5.) That the attachment of side car is not intended for any illegal or unlawful
purpose but solely due to the foregoing reasons; and

6.) That I have executed this affidavit to testify under oath the truth and veracity of
the foregoing statements.
