This is in relation to the loan in the amount of One Million Five Hundred Thousand
Pesos (P1,500,000.00) we availed from FIRST BANK, INC. secured by Lot No. 1969
covered by Tax Declaration No. XXXX PIN No. XXXXX The real estate mortgage was
annotated on Original Certificate of Title No. XXXX as Entry No. 2013000123 and as a
Memoranda on Tax Declaration No. XXXXX.

The loan has been fully settled and a corresponding Cancellation and Discharge of
Mortgage was subsequently issued by your office before Notary Public Atty. XXXXX
docketed as Doc. No. 536; Page No. 109; Book No. XLVIII; Series of 2015.

In this light, and in compliance with the requirements of the Provincial Assessor of
Bulacan, I would like to request for a Certificate of Full Payment from your office
declaring that the loan has been paid in full. This certification is one of the documents
required by the Office of the Provincial Assessor for the cancellation of the memoranda
on Tax Declaration No. xxxxx.
