The Implementation of the LeaderSHAPE Immersion Project

During the actual implementation of the project, the scholar will take note of the activities
carried out for tracking purposes. The template below should be completed everyday or after every
completion of an action under a particular milestone. The scholar should reflect on the lessons learned
for the day or give remarks/observation relative to the completion of an action step. Any revisions of an
activity should be indicated in the reflection.

Reflections refer to the significant lessons learned by the scholar in the project
implementation. Recommendations refer to the actions to be taken in the future in order to
enhance the project’s features for continuous improvement or sustainability.

MOVs, pictures, screenshots, letters/MOA form Donors, recognitions received, approved

evaluation form signed by project evaluators, etc.
