AFFIDAVIT OF UNDERTAKING

______________, Baguio City, Philippines, after being sworn in accordance with law,
hereby depose and state—

1. I am the occupant of a Eighty Four [84] Square Meters along ______________,

Baguio City. We know that the lot owner is a person by the name of _____________.

2. I would like to request for a water connection with the Baguio Water District
[BAWADI]. I hereby acknowledge that the owner of the said lot has the full authority
over the said property and will ask for the disconnection of the said water line once the
property owner will use the said property.

3. I attest to the truth of the foregoing statements to the best of my own personal
knowledge and based on authentic documents and records. I am executing this Affidavit
of Undertaking as well as for all other lawful intents and purposes it may serve.
