Task 1: First-aid arrangements

Question 1
The first-aid needs assessment should consider the following:
 emergency medical service:
According to the scenario it is found that there is no medical services, no availability
of first-aid room instead of first-aid room the injured person was sit down in the
stockroom.
 The absence of medical expertise:
Based on the scenario there is no authorise trained first-aider at stockroom and this
negligence is not noticed by the manager.
 The work force nature:
According to the scenario there are four fulltime workers and two part time workers
that covers the weekends as fulltime workers are not well trained and no training
record have been found in the scenario.
 Working of employers on shared sites:
Bases on the scenario it is found that the workers working inside the stockroom were
not assigned area wised the workers can move anywhere and no proper
communication among workers.
 First-aid provisions for non-employers.
The first-aid treatments were not arranged for non-employers in case of emergency.
 Risk and hazards:
Based on the scenario the stockroom is on the first floor accessed by stairs and the
employers are at high risk of falling down from certain height.
 Distribution of the work force:
In the scenario there are four fulltime workers and two part time workers that cover
the weekends as fulltime workers are not well trained and no training record have
been found in the scenario.
 The accident and illness history of the organisation:
Based on the scenario it is found that there is history and illness record found.
 Organization size:
Based on the scenario that is found that the organisation is found in large shopping
centre of the first floor of the building there is stockroom with a high ceiling that is
accessed by stairs from the back of the store.
 Number of workers in the workplace:
The size of the organization depends on the workers. More workers are required in
the big organisation to facilitate the costumers quick.
 Welfare provisions:
An organisation required to provide the costumers as well as the workers welfare
facilities like washroom, hand wash station and drinking etc.
 Foreseeable emergencies:
In outline of the foreseeable emergency that might occur along with circumstances in
which they might occur and there foreseeable outcome.
 work pattern:
On the basis of scenario it is found that the schedule of work does not have any week
breaks for fulltime workers. Some employers are having short term contracts or some
were hired only for weekends.
 Procedures to be followed:
In the event of a fire for example normal practice as for workers is to exit the building
using the sign escape routes and assemble at a designated place.
 The need of travelling, remote and lone workers:
The first-aid should be available wherever people work so you may need to consider
providing personal first-aid kits or training to travelling, remote or lone workers.
 Procedures raising the alarm:
Workers need to know how the relevant alarm are raised from different emergency

and any escalation procedures to might apply.
 Reviewing the first-aid need assessment:
You should review your first-aid need from time to time particularly if you have
operation chains on your workplace.

Task 2: Management failures and control measures

Question 2 (a)
The following are the management failures contribute to the accident:
1. Lack of management commitment:
There was not proper management system and they don’t now how to implement the
rules of safety on workers as mentioned in the scenario the store manage spent most
of time at the cash register serving customers.
2. Lack of supervision:
There is no proper supervision of work the top management is not contact with day to
day of operation as per the scenario many of the colleges choose to wear fashionable
footwear such as heels open toed shoes and unlessen trainers to word there was no
work to tell them use proper PPE Personal protective equipment
3. Accident reporting:
As per the scenario in the induction training there was not tell someone to work how
to report the accident
4. Lack of train:
The basic training of health and safety is required for any organization but in the
scenario there is not conducted basic any basic training for worker.
5. Lack of emergency procedures:
As mentioned in the scenario there was no proper emergency arrangement when the
worker rushed down the stairs the store manager treat in the injured worker
6. Bad house-keeping:
There was no proper work place control on the basic of the scenario stocked has
been put on the wrong place or left on the floor making at harder to find

The organization does not show or proper team work as for health and safety or team
work is required that in the scenario added point any team work.
8. Lack of documentation:
As per scenario the organization don’t have anything official written down a set of
instruction is needed in written so even I have not time to brief new workers they can
read the instruction before leaving then on their own.
9. Employs turn over:
According to the scenario in the organization employs turnover is quick so the new
temporary workers have lack of experience they cannot handle stock properly and left
the stock room in a mess, on the other hand if they organization here employers for
longer time having more experience they will handle the stock properly easily
accessible.
10. Using worn-out equipment:
According to the scenario the manager neglect the wage of worn-out equipment’s like
broken step leaders.

Question 2 (b)
As from the scenario the accident caused due to the nervousness of the worker and the
find thing that was the root cause of the accident was the wage of heels during work
hour.
 In meeting the first thing that could be discussed to prevent the repeating of such
accident must be of workers must keep good grip on their nerve, they must be
trained to bare situational pressure and to easily over some their nervousness.

 2nd thing that must be discuss would be: The workers must be advised to wear flat
shoes to facilitate their quick movement
 Awareness of the worker about the accident: All the other workers must be
educated about the accident, so they are fully aware of the risk.
 The current accident must be recorded in the accident book for the record.
 The immediate replacement of the broken step ladders.
 Work place inspection: The work place must be inspected regularly in order to keep
the work place well manage.

Question 2 (c)
Hazards communication:
Work and at height including arrangement to avoid the need to work at height and for
safe working at height.
 Lone working:
Arrangement for monitoring there working alone and for emergency to avoid the
repeat of accident we should fallow the following with administrative control.
 Proper Induction Training:
Need of basic health and safety training to worker that would be able how to do work
safety.
 Safety management system:
At the work place the safety management as the basic thing to prevent some on life.
 Safe system of work:
This is a formal procedure which defence with the method of working that eliminated
hazards of minimizing the risk associated with them.
 Reduce exposure:
If the degree which a worker as an export to a hazard can be reduced, then the
worker as far less likely to have an accident with the hazard.
 Supervision:
Referred to management routinely checking workers and exercise their authority to
control behaviour.

Task 3: Reporting the accident

Question 3 (a)
The employees must be reporting the accident to the competent authorities as:
 [The accident reporting to the enforcement authorities is legal requirement under
the 2002 protocol to occupation health and safety convention 1985 C 155 article 4
occupational health and safety convention 1981 C 155 supported by
recommendation 1994.
 It is the legal obligation of the employees to report the accident to occupational
health and safety authorities and civil enforcement local authorities they can
identify were and how the arise risk and how to investigate it.
 The current accident in retail stores must be reported to inform the authorities for
the insurance purpose and claim.
 The local authorities could maintain the record of the incident of the organization to
maintain static and update standards accordingly.
 The accident reporting is important for the civil claims and criminal claims from the
enforcement authorities.
 To ensures the compliance with local and international laws and for the reference
reviews.
 It is necessary for the employers to report the last time injured accident to the
national health and safety authorities to get rid from the legal prosecution, fines
and bans.

Question 3 (b)
The investigations of near messes help us to prevent the accidents as according to
the HSE for every accident there are roughly 90 near messes according to Heinrich
pyramid for every 300 near messes there is one serious accident so if we
investigate the near messes, it will reduce the accident as follows:
 The investigation of near messes will help us identify where existing control
measures failed so gaps are analysed and improvement programmes are
initiated.

causes, and root causes of the particular near messes.
 The investigation of near messes reduce accident and improve safety it help us to
find out what went from, fix problem and learn lessons all too upon it takes an
accident to identify a health and safety problem in the retail and workplace.
 Properly investigation of near messes show us where the risk assessment needs to
review and what are the particular hazards in work place that could be include in
risk assessment and also help the priority the hazard and control measures.
 Near messes investigation increases safety ownership and ultimately leads to
improve health and safety arrangements in workplace.
 Proper investigation of near messes will enable to gather valuable information that
otherwise might not be discussed.

even lives further down the line.

Task 4: Role and responsibility of the manager

Question 4 (a)
The roles of management of responsibilities play an important role to achieve affective
occupational health and safety management system in any organisation and an integral
part of OSHMS.
As in the scenario the manager of the current store was overall lagging in fulfilment of his
role and responsibilities in relation health and safety management system while at some
points his role was effective to health and safety at current store.
In the scenario reflected the current store manager is not much aware about the health
and safety and put responsibilities on the senior workers in the scenario.
As per the scenario the manager however collaborate with senior workers about health
and safety and takes advise on health and safety matters with reflect that the manager
involves the workers which will help affective manage health and safety system.
Store manager should be vigilant to every complaint unless in the scenario.
When it is informed to manager about near messes trips occurred many times store
manager instead of taking immediate actions suggested to have this done whenever
having time for affectively implementations of OHSMS.
Shoe boxes falling from heights, trips over boxes, use of broken step ladder is also a
failure of manager’s incompetency in fulfilling his role and responsibility. Because it is
manager responsibility to provide proper tools when there is any report of default and to
immediately update risk assessments whenever any sort of near misses are reported.

It is also the responsibility of manager to make procedures about proper HSE induction,
reporting near misses, maintaining accident records and providing first aid facility in case
of accident. And scenario shows that there was very poor HSE induction system which
was based on memory and there is no any written updated content available for
induction, while there is no any reporting procedure present inside the store & as far as
accident records are concerned, it was shown in the scenario that records were not
maintained, even manager was not aware of the accident book is present in store or not,
which was emerged from shelf in search of first aid box.
In event of accident in the store it was proved that there was no any reporting procedure
and no any first aid provision which resulted in very poor handling of the incident.
During handling of first aid case it is shown in the scenario that you (senior worker) and
manager tried to handle the case being untrained while these cases are supposed to be
handled by trained individuals.
It reflects there was no any proper trained individuals neither any training system inside
the company nor any mock drills to handle similar kind of emergency and this
responsibility lies on the shoulders of management and in current scenario it failed to
justify it.

Question 4 (b)
The following steps of health and safety competence can improve the current store
manager:
Competency: The competency of manager can improve the following some steps that is
knowledge the manager will have the proper knowledge related to retail store.
Ability: The store manager will have the ability of health and safety to improve their
worker.
Training: the current store manager will have the proper training ability to train their
workers to run the organization.
Experience: The manager must be expert to deal with the employers phenomena.

Task 5: Induction training

Question 5
In the induction pack the following information will be given to new workers:
 The nature of the organisation:
The top management will give an induction training the phenomena of the
organization.
 In the organisation there is no proper health and safety policy top management will
have to make the best health and safety policy for new starters.
 To convey emergency procedures and frequent fire drills to the new workers.
 Detailed knowledge of a specific workplace hazards and control.
 The fire aid facility and use of different fire extinguishers tools usage.
 To know every worker the location of welfare facilities like washrooms, hand wash
stations and drinking water facilities etc.
 To know the new employee the movement around the work place itself and any
other visitors.
 Awareness of incident reporting and procedures to record an incident or accident.
 Every worker should know the general safety role such as no smoking or any
irrelevant practices.
 To know the proper wearing of the personal protecting equipment.
 Every individual have to about the duties and responsibilities of all persons at
workplace.

Task 6: Morality in managing health and safety

Question 6
Morality in managing health and safety:
The following global statics have been published by the international labour organisation
(ILO) as part of their safe work programme (you do not need to remember the actual
figures. We gave them, to highlight the skill of problem.)
 There are over 350000 work related fatal accidents reported each year. Half of
these occur in agriculture.
 Over 2.75 million die every year from occupational accidents and occupational
diseases. Around 2.4 million of these deaths are attributable to occupational
diseases.
 There are over 270 million occupational accidents in 160 million occupational
diseases recorded each year.
 4% the world gross domestic products is last each year through the cost of injury,
death, absence etc.
Other high risk sectors are the construction in fishing industries
These statics indicate that a huge amount of pain and suffering and experience by
people who simply go to work earn a living. The numbers indicate the skill of problem.
What the numbers do not do is till the individual stories. When health and safety is not
managed properly, people can get killed and injured and gruesome ways are suffer
terrible diseases that have massive impact not only on them, but also their
dependents, families, friends and colleagues. This suffering is morally unacceptable.
Employers through management control the premises, equipment and working
practices used by workers to produce the goods and services that the employers
require. Employers, therefore, have a moral responsibility to provide safe and healthy
working conditions.
In simple terms, the moral reasons can be summarised as, it is the right thing to do. It
is right and proper that workers go to work to earn a living return home in the same
state, not suffering from ill health or serious physical injuries. People expect this is a
fundamental right. Workers expect it. Society expects it. Overtime, this societal
expectation has been translated into legal standards.in this way the moral arguments
drives legislation.
The financial reasons for managing health and safety: personal injury incidents,
workers ill health and property damage costs money. In incident or ill health occurs
there will be direct and indirect costs associated with that event some of these loses
can be ensures against, nut many can’t.
Accidents and ill health can significantly affect the financial resources of an
organisation in some case; can put an organisation out of business. This is the
financial arguments for manage g health and safety it is sometimes referred to the
business case because it focuses in money.
When accidents occur the organisation will face both direct and indirect costs.
Direct costs: the measurable costs arising direct form the accident. Example of
direct costs:
 First-aid treatment
 Worker’s sick pay
 Repairs to, or replacement, damaged equipment and buildings
 Lost or damaged products
 Lost production time while dealing with the injury
Indirect costs: Those which arise indirectly as a consequences of the event.
Indirect cost or often difficult to quantify precisely and may be hard to identify.

 Reduction in stop moral
 General difficulties in recruiting staff as a result of the accident
 Damage to public image and business reputation
 Loss of good well costumer following delays in productions and fulfilling

Task 7: Determining organisational factors positively influencing behaviour at


work

Question 7 (a)
 Lack of management commitment the manager was not aware of health and
safety and organization focus on job delivery.
