DEPED COMPUTERIZATION PROGRAM (DCP)

It has been observed that some unfavorable incidents happened in the
implementation of DCP Batches 35-44 in some of our schools. In line with this, an
INCIDENT REPORT FORM was devised to be utilized by school recipients of
per.
2. The form shall be filled-up by schools who had experienced and who shall
encounter future issues and concems regarding the delivery, installation, training,
and acceptance of DCP Packages. The filled-out form shall be sent to the Schools
Division Superintendent through your Division Information Technology Officer
(ITO) together with pictures and other pertinent documents that will support the
issue/concern.
3. The form may be filled-out encoded or handwritten.
4. It is reminded that issues and concems solvable within the level of the Schools
Division shall be addressed as soon as possible.
5. If'the issue and/or concem requires escalation to the region, then the accomplished
incident report form together with the documentations (pictures and/or videos), duly
signed by the Schools Division Superintendent, shall be sent to the Regional
Information Technology Officer at iciu.region2(@deped. gov ph.
6. Ifan issue is escalated to the Region, the Regional ITO is tasked to:
a. Acknowledge the receipt of the incident report
b, Transfer the report to the Team Drive for DCP Incident Reports
¢. Forward the report to the Central Office through the ICTS Director, copy
furnished the Regional Director.
4. Ensure that the issue/concem is acted upon

7. Do take note that threats from the supplier to transfer the packages to other school
recipients, if the schools won’t heed to their request, is not being acknowledged by
the Central Office and shall also be reported as an incident.
8. The Incident Report Form shall also be used as a report mechanism in future DCP
deliveries whenever issues and concerns shall be encountered.
