at________________________, after being sworn to in accordance with law, deposes and says:

That on ______________________, an extrajudicial settlement pursuant to the Rules of

Court was effected between and among the heirs of the estate of the deceased, _____________,
whereby the said estate was adjudicated to________________, now evidenced by Transfer of
Certificate of Title No.______ of the Registry of Deeds of the province of__________________;

That pursuant to the Rules of Court, a bond equivalent to the amount of the subject
property, was filed with the Register of Deeds of ______________, to answer for whatever
claims might be presented against the said estate within two (2) years after the settlement and
distribution of the estate which was on_____________________;

That more than two (2) years had elapsed without any creditors’ claims having been
presented against the above estate, and therefore, pursuant to Rule 74, Secs. 1 and 4 of the Rules
of Court, the said bond may already be canceled.
