SUPPRESSION SYSTEM IN IBM BCRS ROOM LIBIS, QUEZON CITY:

We are pleased to submit to you our quotation for quarterly inspection, testing and maintenance of your FM200 Fire Suppression System. The preventive maintenance program we are offering will be governed by regulations set forth by the following latest National Fire Protection Association (NFPA) code and standards: Also to be considered are the requirements of the "Authority Having Jurisdiction" and good engineering practices. I. SCOPE OF WORKS:

PREVENTIVE MAINTENANCE Conduct quarterly inspection, testing and maintenance: * Please see attached inspection, testing and maintenance program for FM200 Fire Suppression Systems.

Note 1: All inspection and test result shall be submitted not later done 10 days after conducting inspection and testing. Note 2: All replacements parts shall be quoted separately.

OTHERS 1. Provide on-call technical personnel to immediately address and correct any trouble on fire protection system covered in the maintenance. 2. Coordinate with on-duty IT Manager/Supervisor or Personnel any preventive and corrective maintenance work for proper isolation and tagging prior to initiating the works. 3. During the 1st visit, Fsi will conduct training and orientation on its end user / maintenance staff for them to conduct the in-house weekly and monthly inspection as required by National Fire Protection Association (NFPA). II. CONTRACT PRICE:

Based on the above scope of works for a period of One Year, the total contract price is Php 90,000.00 (Ninety Thousand Pesos Only). Notes 3: 1. Terms of Payment: 30% of the total contract price and balance thru progress billing. 2. Our offer is exclusive of Value Added Tax (VAT). 3. Our cost has been computed based on daytime work and continuous access to work area. 4. Any other works not specified in this proposal is on the account of the client. 5.
