What is ASSESSMENT?

- The systematic and continuous collection, organizing, validation and

documenting of data or information.
- A nursing assessment should include the client’s perceived needs, health
problems, related experience, health practices, values and lifestyle.
- In 2008, The Joint Commission established a nursing practice guideline stating
that each client should have an initial nursing assessment consisting of a history
and physical examination performed and documented within 24 hours of
admission as an inpatient.
