The DTTP/TTP/CBTP report document should contain the following

contents:
1. Title page (including the University and college name)
2. Acknowledgment
3. Table of contents
4. Introduction
5. Literature Review (to the point)
6. Justification of the survey
7. Objectives (General and specific objectives)
8. Methods and Materials
9. Results
10.Discussion and Conclusion
11.Lists of identified problems
12.Lists of prioritized problems
13.Action Plan (see the contents of the action plan in the body of this guideline)
Even though it may vary according to the project or organizations interest, the Community
Health Action Plan content should answer the following questions:
1. What is the problem?
2. What are the intended objectives to be achieved, for each problem?
3. Who are the target groups for the intervention?
4. How to achieve objective/s (strategies and activities)?
5. Who will do what in the action plan (responsible person)?
6. By what time you are going to achieve your objective/s? (Time frame)
7. What resources are needed? (budget, manpower, time, etc)
For the sake of summary, students can use tables as following during the presentation.
