AFFIDAVIT OF COMMITMENT

1. That, I am a Faculty member under BS Criminology program of Initao College

located in Brgy. Jampason, Initao, Misamis Oriental;

2. That, as an employee of Initao College, I am expected to uphold the college

charter, its philosophy, vision, mission, objectives and policies;

3. That, I manifest a strong desire and commitment to development of highly

competent and qualified graduates by constantly improving one’s craft, hence commit to
lifelong learning and to continuously update myself with the current trends in education,
likewise to keep abreast with its latest developments;

4. That, I commit to be a member of Initao College’s teaching force for CY 2022 –

2023 and to teach courses under the BS Criminology program, unless I obtain
permission in writing to do otherwise for a legitimate reason and a specific period of
