Affidavit of Explanation

I, Police Staff Sergeant JARRY GLABAN SUAREZ, legal age, married, presently
assigned at Initao Municipal Police Station, Initao, Misamis Oriental, designated as
SUPPLY PNCO / PATROL CAR DRIVER, after having been duly sworn to in
accordance with law hereby depose and say:
1. That, on May 2, 2020, at 8:00 o’clock in the morning, I was on duty at the
Quarantine Control Point (QCP) located at Brgy. Pagahan, Initao, Misamis Oriental and
manned/control the said checkpoint in order to prevent the wide spread of the COVID-
19 pandemic disease;

2. That, around 9:30 o’clock in the morning, received a call from PCpl Melfe C
Agcopra informing me that I will be the one to drive the Patrol Car to Sta Cruz and
Purok Matinabangon, Brgy. Gimangpang, Initao, Misamis Oriental to conduct MCAD
activity in relation to Adopt a Family Program;

3. That, the undersigned immediately informed and asked clearance from PMSg
Rossil T Villardo, Team Leader to go with the personnel tasked to conduct the MCAD
activities;

4. That, the undersigned picked up PCpl Melfe C Agcopra, Asst MCAD/FJGS

PNCO, Pat John Carlo A Galarosa, Asst MCAD PNCO, Pat Chrislyn A Naquin, Beat
Patrol, NUP Laarni Joy J Conadera and NUP Merlyn H Sancho at Cabañesas
residence, P-1, San Pedro, Initao Misamis Oriental;

5. That, the undersigned together with the group immediately headed to Barangay
Gimangpang specifically at Sitio-Sta Cruz and P-Matinabangon for the distribution of
relief goods to less fortunate families and children under the Adopt a Family Program.
The activity ended on or about 11:45 o’clock in the morning.

6. That, the undersigned brought the group back to Initao Municipal Police Station
for a lunch break;

7. That, thereafter, the undersigned immediately returned at the Quarantine Control

Point (QCP) located at Brgy. Pagahan, Initao, Misamis Oriental to perform my task as
duty personnel;
 8. That, on or about 1:35 o’clock in the afternoon, PCpl Melfe C Agcopra called
again and asked me to drive them to Barangay Pontacon to continue the Adopt a
Family activity and gift giving program thereat;

9. That, the undersigned immediately informed and asked clearance from PMSg
Rossil T Villardo, Team Leader to go again with the personnel tasked to conduct the
MCAD activities in Barangay Pontacon;

10. That, the undersigned picked up PCpl Melfe C Agcopra, Asst MCAD/FJGS
PNCO, Pat John Carlo A Galarosa, Asst MCAD PNCO, Pat Chrislyn A Naquin, Beat
Patrol, NUP Laarni Joy J Conadera and NUP Merlyn H Sancho at initao Municipal
Police Station, Initao Misamis Oriental;

11. That, the undersigned together with the group immediately headed to Barangay
Pontacon, this municipality for the distribution of relief goods to less fortunate families
and children under the Adopt a Family Program;

12. That, thereafter, the undersigned immediately returned at the Quarantine Control
Point (QCP) located at Brgy. Pagahan, Initao, Misamis Oriental to perform my task as
duty personnel;

13. That, at around 4:00 o’clock in the afternoon, I informed PMSg Emmerson L
Macasero and PSSg Francis Ferdinand V Dadole, QCP duty personnel that I need to go
to Initao MPS for my daily compliance in Supply Section to be submitted every 4:00
o’clock in the afternoon and headed to the station immediately;

14. That, the undersigned have attached herewith the documentation of said Supply
daily compliance;

That, due to the abovementioned compliance, I was not able to present myself in
Quarantine Control Point (QCP) when the Regional Director visited thereat;
 16. That, the undersigned prays that this explanation will be acceptable and given
merit;

17. Other relief, just and equitable is also prayed for.
