AFFIDAVIT

I, RUTH SALAZAR PUNZALAN, of legal age, Filipino citizen, married, and a resident of
132 Brgy. San Agustin, Trece Martires City, Cavite, Philippines, after being sworn to in
accordance with law, do hereby depose and state:

1. That I am the sole proprietor of a business, I operate under the business R.S.
PUNZALAN MULTISPECIALTY AND DIAGNOSTIC CLINIC and registered under
Department of Trade and Industry (DTI) with DTI registration number 034092040;

2. That due to the growth and expansion of the above said business, I can no longer
manage all by myself, the aforementioned business and as such, I am now
transferring the ownership and management of the above said business, to
PUNZALAN HEALTH CARE INC.;

3. That the transfer of the ownership and management of the above said business to
PUNZALAN HEALTH CARE INC. is not to defraud creditors nor to escape any liability;

4. That in view of the foregoing, I hereby request the Department of Trade and
Industry and the Business Permits and Licensing Office to cancel the Certificate of
Business Name Registration and Mayor’s Permit, respectively, issued in my name
and to issue another in the name of the new owner PUNZALAN HEALTH CARE INC.;

5. That I am executing this affidavit to attest to the veracity of the foregoing and for
whatever legal purposes it may serve.
