GIVING APPROVAL FOR THE COLLECTION AND PROCESSING OF PERSONAL DATA

With my signature I hereby give my approval to Apave Mare d.o.o. – Maritime & Offshore Training
Center for the collection and processing of my personal data for the following purposes:

- registration/application for and attendance at the training course (name, surname, address,
phone/mobile number, e-mail, title/occupation)
- verification of my personal data according to previously determined conditions necessary for
taking part in particular course/training, according to accreditation organization
requirements/conditions,
- issuing and possible sending of certificates following a successfully completed course and
passed exam
- possible subsequent sending of information regarding the certificate expiration and news
related thereto

The Apave Mare d.o.o. is required to protect all personal data from unauthorized disclosure and use
by an unauthorized third party.

In case of taking part in training programme after which an exam is required, your personal data shall
be shared with bodies requiring such data for the registration for an exam and for the issuing of
respective certificates.

Your personal data shall be permanently kept in compliance with legitimate interest of the data
keeping service for the purpose of keeping records on completed training programmes.

By placing your request at any moment, you may request a copy of your personal data, their
correction as well as permanent deletion thereof by cancelling this Statement in writing and sending
the notice of cancellation to contacts specified below.
